Job summary

This is an exciting opportunity to join the Thoracic Aortic Aneurysm Service within Cardiac Surgery at Liverpool Heart and Chest Hospital and gain experience of complex thoracic and thoracoabdominal aortic surgery. The service in integrated with Vascular Surgery through Liverpool Vascular and Endovascular Service (LiVES) and Liverpool Cardiovascular Surgery (LCS).

Main duties of the job

The position is entirely based within the Thoracic Aortic Aneurysm Service with no obligation to the general cardiothoracic middle grade on-call system, clinic cover or theatres.

The post is initially for a 6 month period with a potential for extension to 12 months and in exceptional circumstances to 18 months, after review and mutual agreement.

Experience will be provided in all aspects of aortic surgery from root to aortic bifurcation including open, endovascular and hybrid approaches. National and international candidates are welcome to apply and the post is not restricted to UK National Trainees but candidates should have a certificate of completion of specialty surgical training.

The position is open to candidates with either cardiac or vascular surgical training, preferably towards the end of training or newly appointed at consultant level. There are currently two Aortic Fellows and the exact working arrangements will depend upon background, experience and needs of the appointee.

INTRODUCTION

The Thoracic Aortic Aneurysm Service at Liverpool Heart and Chest Hospital is one of the busiest specialist thoracic aortic services in the UK providing comprehensive care for patients with complex aortic disease.

Six cardiac surgeons with a specialist interest in thoracic aortic surgery operate on more than 300 patients per year.

At the centre of the service is the Aneurysm Clinic which has well over 3000 operated patients under follow-up and another 1000 under surveillance. Patients are discussed at a weekly multidisciplinary team (MDT) meeting which includes cardiac surgeons, vascular surgeons, intensivists and physicians ensuring appropriate bespoke patient centred care.

Facilities include a specialist hybrid theatre allowing hybrid procedures using stents and conventional surgery. A 24/7 specialist aortic on-call service is available 365 days per year.

The service is integrated with Vascular Surgery in the form of Liverpool Cardiovascular Surgery (LCS) with weekly open and endovascular intervention by vascular surgeons. Joint weekly clinics are scheduled for complex intervention. The service is supported by two Aortic ANPs and Aortic Administrator.

We are also involved in research and innovation through collaboration with The University of Liverpool.

Training is important to us and we have had a succession of aortic fellows from around the UK and the world, with both cardiac and vascular backgrounds, who have spent time with our team. In a recent review of the last 5 years of training, Type A dissection repairs were done by trainees. Patients are supported by a specialist Aortic Nurse Practitioner.

We have a Proximal Aortic Fellow and a Distal Aortic Fellow, one supported by Terumo and one by Artivion. If possible new fellows start as a Proximal Fellow focusing on root and arch surgery, then transitioning to the Distal Fellowship, for thoracoabdominal aortic surgery experience. The distal fellow works closely with our Vascular colleagues. It is expected that the two fellows will work collaboratively to cover on-call, clinics and theatre with agreement on who would benefit most from opportunities. Although typically the fellows are cardiac surgery trainees, on occasions we have appointed vascular surgery trainees.
