Job summary

This is an exciting opportunity to join Liverpool Heart and Chest Hospital, the largest single site specialist heart and chest hospital in the country. We are seeking applications for a senior clinical fellow in coronary intervention post which is available from August 2024.

Main duties of the job

The post will be ideally suited to post-CCT trainees who are seeking further subspecialty experience in PCI before taking up consultant posts. The post is available for 12 months. The appointee will be expected to have completed their core training in cardiology, and have substantial experience in routine PCI. They would be expected to participate in the tier 2 on-call rota as part of the post. This post is ideally suited to a senior trainee in interventional cardiology who wishes to consolidate their knowledge and increase their experience in Complex Coronary Intervention before taking up a consultant post. Applications would also be welcomed from individuals with less experience who are committed to a career in cardiac intervention. The appointee will be expected to have good experience in the performance of modern cardiac interventional procedures, as well as extensive experience of basic cardiac catheterisation.

JOB SUMMARY

The post is full time at the Liverpool Heart & Chest Hospital. The successful applicant will work under the supervision of the 11 interventional cardiologists at the trust and also for the joint appointees. There will be exposure to all aspects of clinical cardiac intervention, with a focus on complex intervention. Outpatient work will involve general cardiology and pre-assessment for day-case PCI. The appointee will be jointly responsible for management of patients transferred to the LHCH with acute coronary syndromes and take part in the primary PCI programme.

The successful candidate will receive intensive training in all aspects of interventional cardiac intervention including rotablation, lithotripsy, LASER atherectomy, IVUS, OCT, pressure wire, embolic protection, thrombus aspiration and coronary sinus reducer. They will participate in the hospitals CTO programme. It is anticipated that suitable candidates would be independent operators in all but the most complex cases certainly by the end of the twelve-month post.

There will be regular emergency cover as part of the cardiology SpR rota which will be closely supervised by consultant staff. The post-holder will gain considerable experience of the management of cardiac patients in a tertiary setting. There is a full shift rota in place, 1:15 band 1a inclusive of prospective cover.

There will be a commitment to contributing to the rota for ACS PCI weekend lists alongside the on-call interventional consultant. This is separate to the on-call commitment and will provide a valuable training opportunity.

Specific duties

Outpatient clinics- Two clinics per week

Available for pre assessment

Catheter Lab - Up to six catheter lab sessions per week with attendance at further sessions for specific advanced training. Priority access to fortnightly CHIP list as well as allocation to complex operators

Ward Work - The trainee will undertake registrar of the week role on a rotating basis during which time they will supervise the day to day work of the senior house officers/trust doctors in conjunction with the consultant of the week.

GENERAL STATEMENTS

Liverpool Heart and Chest Hospital (LHCH) provide medical and surgical adult cardiothoracic services to the people of Merseyside, North Wales, Isle of Man, parts of Lancashire and Cheshire.

This catchment area has a population of 2.8 million people.

The Centre provides a full range of invasive and non-invasive diagnostic cardiac and pulmonary facilities. Thirty sessions of coronary and structural intervention are undertaken per week. During the last year 2300 PCI procedures and over 2500 invasive coronary diagnostic procedures (including pressure wire/IVUS/OCT) were undertaken. There are 6 TAVI lists, 2 structural (ASD/PFO closure, paravalvular leak closure) lists, 1 LAAO closure and 3 ACHD intervention lists per month.

The Centre is a foundation trust, which has seen a great deal of development in the last few years. As a result of major capital investment the Centre is able to provide:

  • 180 medical and surgical beds
  • 26 day case beds
  • 12 bed cystic fibrosis ward
  • 10 CCU beds; 24 surgical ITU beds (potential to expand to 30)
  • 8 dedicated cardiothoracic surgical operating theatres
  • Endoscopy suite
  • Recovery room
  • X-ray department with a multi-channel spiral CT Scanner and MRI scanner
  • Pulmonary function and oesophageal laboratory

Research participation is expected and the post-holder will be given a project to carry out with a view to publication within the time frame of the fellowship. Attendance at national and international meetings will be encouraged, particularly for presentation of data.

The centre undertakes teaching of all final year under graduates from the University of Liverpool Medical School and is also recognised for the training of nurses and other professional and technical staff. The post-holders will be involved in teaching these groups of staff and other junior doctors.

A regular programme of conferences, lectures and seminars takes place within LHCH. The centre is committed to the principal and practice of clinical audit in which all medical staff are expected to participate. The appointees will be expected to attend and take part in the various educational meetings. They will be expected to present cases, develop an audit interest and present the data locally.

A specialist library is available on site. In addition, the libraries of the Liverpool Medical Institution, Liverpool University and Broadgreen Hospital are also available for reference. There is also a purpose built doctors mess, including office space, computing facilities and recreational facilities.

RECORD OF TRAINING

All training is subject to annual assessment by an educational supervisor and the programme director. Educational supervisor will be assigned at the start of the fellowship and will provide close supervision to ensure training goals are obtained. It is expected that the supervisor will be one of the complex PCI operators.

As well as serving as a review and training, this will allow planning and flexibility within the post-holders programmes and will allow for career advice and development of specific interests accordingly. Although not within a formal training programme the disciplines of the usual SpR commitment will be applied.

Post-holders will be expected to keep a log book of all practical procedures undertaken, which will be regularly reviewed by their supervising consultant and signed accordingly.

ANNUAL LEAVE AND STUDY LEAVE

The post-holders will have full entitlement to annual and study leave. All leave arrangements are subject to approval by the administrative SpR and the supervisory consultant. In addition, study leave applications are also subject to approval by the Postgraduate Clinical Tutor.
