ACCOUNTABLE TO: Cardiac Rehabilitation Team Leader and CVD Matron
JOB SUMMARY
To provide a high standard of clinical exercise physiology, working as an integral part of the Community Cardiovascular Rehabilitation Team delivering a menu based programme to the Residents of the Borough of Knowsley. You will deliver the programme in a variety of community settings, with specific emphasis on the delivery of exercise testing, prescription and delivery.
PRINCIPAL ACCOUNTABILITIES
CLINICAL RESPONSIBILITIES
1. Cardiovascular Rehabilitation Programme: To work effectively as part of the multidisciplinary team ensuring the delivery of high quality exercise based rehabilitation
a. Monitoring and prescribing the exercise component based upon sound evidence and best practice guidelines.
b. Be responsible for the patient suitability assessment, test facilitation and prescription of appropriate exercise for this group of patients
2. To continue the development of an innovative programme of cardiovascular rehabilitation which gives patients genuine choice to support their recovery and self-management, including supporting carers.
3. To assess complex cardiovascular patients, develop and deliver an individualised programme of rehabilitation according to patient goals.
4. To support the integrated rehabilitation collaboration with the Knowsley Community Respiratory Services.
5. To deliver a range of exercise based education sessions using a variety of teaching methods on an individual and group basis suitable for the patients medical, social, psychological and cultural factors, which are evidence based.
6. Communicate with service users such as patients, carers and staff, the public and their representatives, managers and colleagues ensuring effective and accurate information. Ensure confidentiality at all times.
7. Ensure appropriateness of exercise monitoring and exercise response based upon clinical knowledge and evidence base, ensuring adequate levels of supervision of all exercise sessions in line with policies and procedures.
8. Ensure that all patients accessing the exercise component of rehabilitation are offered appropriate care via an exit strategy for promotion of continued lifestyle change within their local community
3. MANAGEMENT RESPONSIBILITY

3. Be responsible for undertaking risk assessments prior to delivering services.
4. Report any concerns, requirements for action to the Cardiovascular Rehabilitation Facilitator
5. Regularly check and monitor any exercise and resuscitation equipment as per protocol, ensure that equipment is maintained to standard and safe for patient use
6. To be responsible for maintaining own competency to practice, through CPD activities and to maintain a portfolio which reflects personal development.
7. To attend mandatory training in line with trust policy
8. Participate in the staff appraisal scheme as an appraisee and be responsible for complying with your agreed personal development programmes, tutorials, individual training sessions, external courses and peer review
4. EDUCATION AND TRAINING
- Facilitate the transfer of information to appropriate external parties
- Participate in the development and delivery of education programmes for generic staff, patients and carers
- Mentor students on placement, identifying learning needs and giving ongoing support
5. PROFESSIONAL RESPONSIBILITIES
1. To undertake all aspects of clinical duties as an autonomous practitioner, including professional and legal accountability and managing clinical risk for all aspects of own work. Some aspects of work will involve delivery of the care as an individual practitioner or as part of a multi-disciplinary team
2. To undertake as directed a collection of data for use in service audit and research projects. To manage and undertake audit & research in specific areas of clinical practice and service delivery using a range of research methodologies as part of MDT. Make recommendations to clinical lead/manager.
3. To ensure adherence to safety guidelines, patient care and consent guidelines.
4. Leading in the delivery of the induction process, formalising entry to the exercise area in line with health and safety guidelines
5. To maintain systematic clinical and professional records as required by the service
6. To participate in the effectiveness of communication within the service by attendance at clinical, team brief, team meetings as advised by Line Manager
6. RESPONSIBILITIES COMMON TO ALL STAFF
1. To establish, maintain and develop professional working relationships with colleagues
2. Adhere to trust guidelines regarding patient consent and confidentiality of information about patients, staff and health service business in accordance with data protection act.
3. Follow all agreed Quality Assurance Mechanisms in operation and contribute generally to the establishment and development of a quality service.
4. Adhering to LHCH policies and procedures including standing financial instructions and orders.
5. Participate in the Trusts Development Review Appraisal process and, with line management, objectives and development plans. All staff are entitled to participate in the Review Process and, with their manager, are jointly responsible for the completion of agreed actions.

12. Staff who come into contact with children, vulnerable adults, parents and carers in the course of their work and/or have access to records will have responsibilities to safeguard and promote the welfare of children, young people and vulnerable adults.
13. Staff who come into contact in the course of their duties, with parents, carers or other significant adults or children, young people and vulnerable adults should always be mindful of safeguarding and promotion of the welfare of these individuals.
Staff will be trained to a level appropriate to their responsibilities in this respect in line with the current KSF post outline and the ongoing PDR process.
