Job summary

We are looking for a motivated, dynamic, and innovative individual capable of leading a team, implementing change and representing pharmacy internally and externally to become our advanced clinical pharmacist responsible for pharmacy education and training across Mersey and West Lancashire Teaching Hospitals NHS Trust (MWL). As lead, the post holder will be responsible for leading the development, management and delivery of the education and training programs for the whole pharmacy team .In addition, they will support the Education Lead within the Learning and Development department with the strategic development and delivery of medicines related training to nursing and medical staff in the Trust and support development of medicines related training to undergraduate pharmacy student teaching that is delivered in the Trust.

The postholder will need excellent interpersonal and communication skills and the ability to organise and prioritise their work. They will also require previous experience, knowledge and understanding of the training requirements and objectives of undergraduate, trainee and advanced clinical pharmacists and how training is delivered effectively to individuals.

The successful candidate will be based at Southport Hospital and will be expected to participate in weekend and late duty rotas and pharmacy on call.

Main duties of the job

The post holder will:oBe responsible for leading the development, management and delivery of the education and training programs for the whole pharmacy team (>250 staff). This will require upskilling of pharmacy staff to take on extended and new roles within the Trust and in the Primary Care setting.oSupport the Education Lead within the Learning and Development department with the strategic development and delivery of medicines related training to nursing and medical staff in the Trust.oSupport development of medicines related training to undergraduate pharmacy student teaching that is delivered in the Trust.oProvide a specialist clinical pharmacy service to patients and staff in an appropriate speciality.The postholder will work with the clinical specialist pharmacists and clinical services manager and other senior members of staff to develop clinical and other services within the Trust.The postholder will provide a clinical pharmacy service to designated wards.

KEY DUTIES

Lead the strategic direction and implementation of education, training and continued professional development of all pharmacy staff within the Trust.2. Develop, manage, implement and evaluate education and training policies for the pharmacy department.3. Design, implement and review the trainee pharmacist training program. 4. Prescribing of patients regular medication on admission and other appropriate drug treatments according to clinical pathways within the departmentd. Assist in expediting the discharge process by prescribing on the electronic discharge systeme. Initiation of monitoring where deemed necessaryf. Review and update of care plans depending on response, results of investigations and adverse effectsg. Therapeutic drug monitoring for specific drugs15. Carry out a daily ward round to clinically review individual patients treatment and to determine the most appropriate choice of medication in response to complex series of data relating to their condition, their response to treatment and reduction of avoidable adverse events.

Ensure that all patients understand how to take their medication prior to discharge, and counsel any patients when necessary, particularly those taking complex drug regimes, those who are anxious or with physical or mental disabilities and those with language difficulties. Supervise the assessment of patients for the use of compliance aids to reduce the risk of medication-related errors particularly where this may have been a contributing factor the hospital admission.17. Provide appropriate verbal or written information to patients or their relatives about their medicines when necessary, particularly where this may influence the success of treatment once the patient has been discharged.18. Liaise with the primary care sector to ensure follow-up of pharmaceutical care in the community for patients needing additional pharmaceutical support, such as compliance aids.19. Work with medical, nursing and the site management team to facilitate patient discharges to maximise patient flow.20. Liaise with medical and nursing staff to ensure the timely supply of medication to inpatients to minimise missed doses and to facilitate the discharge process by ensuring the timely availability of medication.21. Develop evidence-based treatment protocols within specialty for use within a Directorate or across the Trust where appropriate.22. Facilitate implementation of local and national guidelines where appropriate.23. Work closely with Consultants, Nurses and other multi-disciplinary team members to ensure the safe and appropriate use of medicines.24. To work with the Clinical Pharmacy Services Manager and other senior members of staff to develop pharmacy services, education and training as appropriate.25. Attend clinical ward rounds, clinical and multidisciplinary meetings as appropriate.26. Advise clinicians of all levels to influence treatment decisions that will lead to improvements in patient care.27.
