Children's Community Nurse

Job summary

We are seeking a dynamic and motivated nurse to join our Children's Community Nursing Team in the Community of St Helens and Halton areas.

You would be based in Lowe House HCRC in St Helens and travel across the boroughs.There will be lots of opportunities for learning new skills; the successful candidate must be comfortable in working in patients homes, liaising withother hospitals and preparing care plans for children in the community. The role will also involve providing nursing intervention for children who require complex and life limiting care alongside the delivery of up to date, evidence based training.

Main duties of the job

  • Support with provision of training in line with the identified package of care for the ICP funded child/young person.
  • Deliver evidence based clinical education and training to education and support staff, to support the care of children and youngpeople within the children's community nursing team caseload.
  • Plan and deliver individual programmes of specific care for children within a range of care and educational settings, who have avariety of healthcare needs in a competent manner that recognises own limitations and seeks appropriate supervision.
  • Possess advanced clinical skills to closely monitor the child's progress and improvement and to detect deterioration, promptlyfollowing the agreed care pathway to medical reassessment.
  • Deliver specific health education, health promotion and illness prevention advice to fellow service providers and children/families.Working in collaboration with public health school nurses.
  • To respect family's cultural, spiritual and religious practices and work in partnership with families in promoting parent participationand child/family centred care.
  • Participate in the proactive planning process for the transitional period, prior to young people transferring into adult care services.
  • To manage own caseload and ensure effective management and time keeping
