Job summary

15 Hours Per Week 12 month Fixed Term/Secondment

An exciting opportunity has arisen to work as part of the St Helens Community Cardiac Service. Experience of working within the cardiology environment, with evidence of clinical knowledge and skills of caring for cardiac patients is essential.

The role involves supporting patients following a cardiac diagnosis such as an MI, cardiac bypass surgery or heart failure, to assist in the patient's recovery, improve their health and wellbeing by providing evidence based clinical practise and knowledge. The role also includes supporting the therapy team in delivering a cardiac rehabilitation exercise programme.

The successful candidate will work in as part of a nurse led service working alongside the cardiac nurses, heart failure nurses and therapy staff undertaking a variety of duties. Potential candidates are welcome to contact the service to discuss the role in more detail.

An Enhanced DBS with barred lists for Adults is required for this post.

Main duties of the job

  • To assess, plan, implement and evaluate care for cardiac patients with a variety of cardiac conditions.
  • To work autonomously and manage own specialist defined patient caseload.
  • To deliver cardiac rehabilitation programmes phase 11 and 111.
  • To be involved in developing specialised, evidence-based service delivery which is linked to both local and national targets.
  • Liaise with all members of the MDT involved in patient care including GP, Cardiologists, etc.
  • Provide clinical supervision and mentorship.
  • Complete a holistic assessment of risk factors and offer appropriate lifestyle advice includes exercise and educational component as well as psychological support and lifestyle modifications.
  • Refer patient to appropriate services for ongoing care.
  • Deliver nurse led clinics and home visits post cardiac event clinics where patients recovery symptoms and treatments are assessed.
  • Advise/undertake drug titration and post MI clinics
  • Collection and input of NACR (National Audit of Cardiac Rehabilitation.
  • Work in collaboration in the development and delivery of coronary heart disease training for the Primary Healthcare Team

We provide the Mersey Regional Burns & Plastic Surgery Unit at Whiston Hospital and the Spinal Injuries Unit at Southport Hospital to more than 4 million people across the whole of Merseyside, West Lancashire, Cheshire, Isle of Man and North Wales.
