Job summary

An amazing opportunity has arisen within Children and young peoples continuing care team. The service offers home based based care and community support to clients who have complex medical needs.

As a Senior Team Leader you will be involved in the clinical supervision and ongoing support of our team of Senior Clinical Support Workers and Band 5 nurse colleagues. The role will involve supervision and training and therefore requires a good level of clinical experience.

Responsibilities also include planning, implementation and evaluating care in conjunction with parents, carers and other allied health, education and social care professionals. You will be expected to case manage a designated group of clients. Therefore the role requires excellent communication skills, both verbal and written.

You will be working with children and young people with various levels of dependency. All the children/young people you will be working with have complex health issues/severe disabilities. The service provides bespoke care for a wide range of conditions which range from ventilator dependent young people, terminally ill children and others with severe disabilities/additional complex health problems.

This can be a very challenging, yet extremely rewarding role. You must have a genuine interest in the welfare of children, a common sense approach and display excellent interpersonal and communication skills.

Main duties of the job

  • Identify children with complex health needs and undertake assessments under guidance of the National Framework for Children and Young People's Continuing Care.
  • Work in collaboration with children and young people and families to understand their preferences in regard to care.
  • Lead on holistic assessment of the child/young person and the family.
  • Act as the Lead Health professional to correlate reports/risk assessments from the MDT in respect of bespoke packages of care.
  • Convene and lead Multi-Disciplinary Meetings to plan care and ensure the needs of vulnerable children/families are met. Ensure continuity of care by appropriate sharing and transfer of information.
  • Act as Health Assessor in completion of the Decision to Support Tool.
  • Attend Children's Commissioning panel and make recommendations around funding arrangements.
  • Participate in safeguarding meetings in accordance with Trust and local Safeguarding Children's procedures, policies and guidance.
  • Be actively involved in the training process for parents, carers and other professionals to ensure competency and evidence-based practice.
  • Participate in common assessment framework CAF/Team Around the Child TAC process following integrated working guidelines.
  • Participate in the development of evidence-based services.
