Job responsibilities

Effectively manage a caseload and where appropriate, delegate care to non-registrants and maintain professional accountability for supervising that care

Ensure effective communication with babies, children, young people, parents, carers and others including those whose first language is not English, liaising with and co-ordinating interpreting services.

Undertake holistic health and Our Family Family Help assessments and plan, implement and evaluate evidence-based nursing care for individual babies, children, young people and their families.

Support the development, planning, implementation and evaluation of health education and promotion packages using evidence-based materials and methods.

Where appropriate, contribute to multi agency processes to assess and develop care plans to support babies, children and young peoples health and wellbeing

Have a good knowledge of local services in health and partner agencies and where appropriate, liaise and refer a baby, child or young person to multi-disciplinary teams and other specialist services.

Take on the role of lead professional to coordinate delivery of care plans for children and young people e.g. Children on Family Help plans.

Promote team working at all times and act up, as directed in the absence of the clinical team leader. Provide line management duties for junior members of staff within your locality team e.g. 1-1s, PDR, caseload supervision.

To develop in conjunction with Deputy professional Lead, implement, monitor and evaluate quality standards e.g. SOPs with HCP 0-19 Practice, utilising the clinical governance framework to support the provision of a high quality, safe care. This includes contribution to reports for service monitoring e.g. commissioner reporting.

Provide pre, post registration student nurses and staff requiring induction, mentorship and preceptorship with relevant and suitable experiences within children, young people and familys public health service, to include teaching and assessment throughout their placement.

Participate in the continued development of the children, young people and familys public health service, to include meetings and close liaison with clinical lead and to work in partnership with other health care professionals within the Trust and other agencies.

Take responsibility for and effectively manage a lead area of the Health Child Programme, as agreed with the Professional Lead.

Represent the service on a range of multi professional forum as requested and agreed with the Professional Lead

In the absence of the clinical chair team meetings to cascade information and plan delivery of the team workload in the absence of the clinical lead

Constructively participate in nursing, clinical and managerial meetings as required

Monitor and report to the clinical lead regarding client and staff safety, accidents and reportable incidents as necessary in line with risk management local and national policies

Contribute to the planning of staff training, both mandatory and for professional development, in conjunction with clinical lead and FFCP Health coordinator.

In consultation with clinical lead and FFCP health coordinator influence and manage change within the service utilising research/evidence-based practice and audit mechanisms.

Working in partnership with the multi-agency team provide clinical advice, information and support regarding babies, children and young people with long term conditions, disability or specific health problems to minimise negative impacts on their lives and promote health gain and social inclusion to the children, family and carers.

Support the development, planning, implementation and evaluation of health promoting activities with individuals and groups, utilising evidence-based materials and methods e.g. sexual health and relationships, emotional health, growth, healthy lifestyles

Develop and maintain effective communication networks and work collaboratively with key partners, health workers, statutory and voluntary agencies to actively promote national and local public health priorities

Plan, deliver and evaluate group work for children and young people and/or parents e.g. emotional Health groups, parenting groups

Plan, deliver and evaluate training sessions to early years settings, schools and other agencies.

Act as role model and FFCP leader leading by example through practice with positive attitude and presentation.

Attend regular team and service meetings to give and receive relevant information and to maintain effective team working.
