Job summary
An excellent opportunity has arisen for a band 6 Anticoagulation Nurse to work as a member of the Anticoagulation Team within the Pathology Department. As a valuable team member you will carry out the analysis and interpretation of technical investigations of clinical samples in order to dose patients according to the current guidelines for anticoagulation.
You will be responsible for clinical assessment of patients - indications for therapy, the therapeutic ranges and necessary duration of therapy. You will undertake Anticoagulation clinics in multiple settings including the hospital, community and patients' homes. You will contribute to influencing all aspects of care and management of patients within the specialty and promote the delivery of high quality, clinically effective care.
Requirements:
Registered Nurse of the appropriate part on the NMC Register
NMP Qualification.
Proficient in Phlebotomy
Previous experience within an anticoagulation setting
Excellent communication skills and interpersonal skills
Driver prepared to travel throughout the borough of Walsall
Main duties of the job
To monitor and dose patients in the Anticoagulation clinics and in patient's own homes and to refer to other clinicians or services.
To educate newly referred patients and their carers, in aspects of anticoagulation management and monitoring, who attend inpatients and community-based clinics.
To carry out diagnostic testing using Near Patient testing devices such as a Coaguchek (or similar) in order to obtain the INR result.
To prescribe anticoagulation therapy in accordance with local guidelines and following appropriate assessment of patients INR or creatinine clearance status.
You will be responsible for the initiation of oral anticoagulation for differing conditions and ensuring correct bridging with LMWH is followed when circumstances arise.
To offer advice and information on request, on issues associated with anticoagulation management and monitoring, from other clinicians, patients and carers.
To undertake phlebotomy as required in order to obtain venous blood samples.
The post holder will contribute to influencing all aspects of patient care and management of patients within the specialty and promote the delivery of high quality, clinical effective care, prioritising and utilising research in every day practice providing specialist advice to patients.
