Job summary

This is an exciting opportunity to join our IBD Clinical Nurse Specialist team. This is advertised as a band 7 post, however we are willing to support junior staff in progression with their career therefore, we can offer a band 6 post for 12-18 months as a developmental post.

Main duties of the job

The post holder will be responsible, through nurse-led clinics, for the specialist assessment and management of a significant caseload of patients with inflammatory bowel disease.

The post holder will undertake out-patient clinics and review inflammatory bowel disease in-patients as required.

The post holder will be a highly experienced inflammatory bowel disease nurse who uses specialist knowledge and skills to lead the improvement of inflammatory bowel disease services between primary and secondary care.

The post holder will be able to take a relevant history and clinical assessment to develop a management plan and evaluate the progress of that plan within the limitations of their knowledge and skills while being supported by medical and other members of the multi-disciplinary team. They will demonstrate clinical judgement and critical decision-making skills based upon established guidelines.

The post holder will work under the supervision of the Consultant Gastroenterologists proactively developing professional practice, demonstrating evidence of increasing autonomy, clinical judgement and decision-making skills and contribution to the development of others.

As part of the multidisciplinary team, and under the supervision of the Consultant Gastroenterologists, the post holder will utilize their specialist knowledge and skills to:

Provide an effective inflammatory bowel disease service by receiving direct referrals.

Assess, diagnose, plan, implement and evaluate treatments and interventions for a variety of inflammatory bowel conditions.

Undertake relevant clinical examination as indicated by the patients condition; analysing clinical signs and investigation results to support diagnosis and initiating any treatment as required.

Using expert clinical judgement, evaluate patients physiological response to treatment, discuss any issues of concern with consultant medical staff and implement appropriate course of action.

Undertake the role of Independent Nurse Practitioner within the clinical setting of and in line with Trust policy/national guidance.

Maintain accurate and up-to-date confidential and timely patient records, ensuring that all patient documentation (both written and electronic) is in line with agreed Trist standards.

To provide effective teaching to patients and families/carers in specific inflammatory bowel disease management or treatment.

Act as a resource for staff advising on local, Trust and national policy, procedures and guidelines ensuring patient safety and clinical governance.

Understand and apply the legal safeguarding framework that supports the identification of vulnerable and abused adults/children and be aware of statutory vulnerable patients health procedures and Trust guidance. Ensure appropriate referral if required.

To organize and facilitate the inflammatory bowel disease MDT and biologics review and complete relevant blue tech documentation in conjunction with the multidisciplinary team.
