We are seeking a dynamic, self-motivated, enthusiastic nurses, looking for the opportunity to develop their nursing career working within one of our integrated community nursing locality teams in Walsall.
The role involves the delivery of holistic nursing care to patients within their own homes, care homes and clinic environments.
You will provide high quality clinical care, assessment and treatment for patients, actively promoting self-care and promoting independence.

Main duties of the job
To work within the 'Place Based Teams Care Group' within the Out of Hours community nursing locality teams.
You will cover the borough of Walsall between the hours of 18:00 - 08-00
To undertake clinical assessments, implement and evaluate treatment plans, supporting patients in such areas as wound care, catheter, continence care and End of Life care. Demonstrating evidence based/best practice ensuring quality service delivery and appropriate admission avoidance. Support to junior and untrained members of staff within the team.
To adhere to Walsall Healthcare NHS Trust policies, procedures and protocols to promote the quality of patient experience in the community, ensuring the highest standard.

About us
Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 284,300. It provides inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor houses the full range of district general hospital services under one roof.
