Under the direction of the Urgent Community Response Nurse is charge you must assess, plan, implement and evaluate patient care demonstrating evidence based / best practice, considering own clinical confidence and competence. You must implement enhanced programs of care
To be part of the Urgent Community Response Team to provide high standards of clinical intervention and to provide nursing care for service users accessing the range of services this includes acting as nurse co-ordinator as and when required.
To implement enhanced clinical care programs that are of the highest quality & standards and in line with the Walsall Healthcare NHS Trust's policies and procedures
To work within a multi-agency / multi-disciplinary team adopting a 'no blame' culture

Main duties of the job
To be responsible for the assessment of care needs and the development, implementation and evaluation of programs of care, within Scope of Professional Practice.
Demonstrate high level of judgmental/analytical skills when interpreting and performing patient assessments and care planning.
Maintain accurate and up to date nursing records, maintaining patient confidentiality, and statistical records as required by the service.
Work with other professionals to promote multidisciplinary working.
Develop clinical skills and access training as required / directed to enable participation in future service developments.
Assist in identifying the physical resource needs of the team and contribute to plans for improvements and maintenance of equipment in respect of available resources.
Assist in the supervision of qualified and unqualified staff and facilitate their continuing professional development.
Ensure effective learning experiences and opportunities to achieve learning outcomes for students and the provision of learning opportunities.
To prepare lesson plans, deliver training programs to the multi-disciplinary team members, and to review and elevate these programs.
Communicate information to patients and relatives with empathy and reassurance.
