Care Navigation Centre is a well-established team in Walsall providing a single point of access for professionals seeking to keep patients in their own home or to avoid an ED admission.

Main duties of the job
An exciting opportunity has arisen for a Clinical Sister / Charge Nurse / Clinical Practitioner to join our Care Navigation Centre. You must be a registered practitioner (nurse / paramedic) with evidence of post registration practice at Band 5. Experience of multi-disciplinary / multi agency working is essential. You will be part of a team and have responsibility for the triage and onward referral of patients who require support from the plethora of community services available in Walsall. You will need to work flexibly to meet the demands of the service and be able to forge strong links with colleagues from the community and acute services. You should have good IT skills and a pleasant telephone manner.

Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 284,300. It provides inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor houses the full range of district general hospital services under one roof. The £170 million development was completed in 2010, and ongoing upgrades ensure the Trust now has state-of-the-art facilities, including a Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit.
