Job responsibilities

Leadership

Assess, plan, delegate, maintain and review caseloads for each team member, identifying needs and skill mix to ensure delivery of service. Analyse and respond to situations at team and one to one level taking account of changing priorities at team and 1-1 level

Represent the service on a range of multi professional forum as requested and agreed with the cluster lead.

Organise and chair weekly team meetings to cascade information and plan delivery of the team workload in the absence of the clinical lead.

Constructively participate in nursing, clinical and managerial meetings as required.

Monitor and report to the clinical lead regarding client and staff safety, accidents and reportable incidents as necessary in line with risk management local and national policies.

Act as a role model, lead by example through practice with positive attitude and presentation

Contribute to the planning of staff training, both mandatory and for professional development, in conjunction with clinical lead.

Undertake team members personal development review and clinical supervision, ensure that individual staff development plans are formulated and objectives agreed in line with corporate objectives.

In consultation with clinical lead, influence and manage change within the service utilising research/evidence based practice and audit mechanisms

Develop and maintain effective communication networks and work collaboratively with key partners, health workers, statutory and voluntary agencies to actively promote national and local public health priorities.

Demonstrate up to date knowledge on legislation and national directives relating to the service is obtained through regular updates from the steering group, Walsall Healthcare NHS Trust, Internet and other sources.

Ensure effective communication with the child, family and others including those whose first language is not English, and LD liaising and co-ordinating interpreting services as appropriate.

Maintain direct communication and liaison with the clinical lead and attend regular meetings

Provide written and verbal detailed information through comprehensive assessment of the child and family to facilitate referrals to specialist services, e.g., Child and Adolescent Mental Health Service (CAMHS).

Work collaboratively to provide antenatal support to individuals assessed as needing early intervention.

To liaise with teaching staff and school medical staff as appropriate, acting as a link between school and home as necessary.

Clinical Skills

Work in close liaison with the designated and named nurse for child protection and the co-ordinator for looked after children.

Work within Walsalls Safeguarding Childrens Board guidelines and local child protection policies and procedures.

Initiate and chair or participate in multi-agency meetings to plan care for an individual child in need.

Working in partnership with families and other agencies contribute to the assessment framework for children in need / Early Help processes.

Critically analyse information gathered from the child health records and other relevant sources and provide accurate reports for formal presentation, e.g., case conferences.

Work as a member of a multidisciplinary team providing professional health advice in formulating, implementing and evaluating the child protection plan. Attend case conferences and core groups.

Participate in personal clinical supervision with child protection supervisors.

Within the scope of professional practice and using appropriate frameworks, take lead responsibility to assess, plan, deliver and evaluate the health needs of specific groups of children e.g., looked after children, children with medical needs. This may include undertaking nurse led clinics and assessments.

In conjunction with child and family, liaise with members of multi-disciplinary teams. Refer to other specialist service and engage in joint planning with child, family and other identified services.

Liaise and communicate with other services to assess and produce individual care plans and to co-ordinate their involvement in supporting the child in school or other settings.

To promote the need for immunisation and to advocate the importance of prophylaxis care. To undertake the immunisation of children in line with Trust procedures as required.

Use an assessment framework and adopting a team approach, undertake or delegate as appropriate the 8 weeks, 9 month and 27 month assessments with professional support to nursery staff.

Be identified as the Named Health Visitor for child protection cases, supporting and supervising team members with child protection issues as required.

To undertake the role of Nurse Prescriber within the scope of the Nurse Prescribers formulary and the Trust Guidelines following the appropriate evidence based information for a range of conditions.

Utilise the Promotional guide or Post Natal Scale to communicate with clients implementing a plan of care in partnership with families in need/crisis.

To develop, monitor and evaluate quality standards with Health Visiting Practice, utilising the Clinical Governance Framework to support the provision of a First Class Service. Develop, monitor and evaluate quality standards within the Health Visiting Service. Incorporate clinical guidelines/service protocols, in practice to ensure that best evidence based care, is delivered to the client group.

Person Specification

qualifications and experience

Essential

• First Level Registered Nurse
• Specialist Community Public Health Nurse (Health Visiting)
• Demonstrable experience working with children and young people
• The ability to make decisions which are timely, effective and appropriate
• Able to establish and maintain key relationships with colleagues and other disciplines both internally and externally

Desirable

• Demonstrable or relevant experience in delivering health promotion and health education programmes
• Effective communication skills both written and verbal with children, families and others

knowledge and skills

Essential

• Evidence of utilising evidenced based practice and appraising standards, providing clinical direction and support for staff
• Detailed knowledge of health & social care policy relating to childrens services

Technical Skills

Essential

• Basic keyboard skills and ability to use microsoft packages
• Ability to access information using internet
