It is an exciting time to join the Health Visiting Service within Walsall Healthcare NHS Trust. The service has recently been transferred into the Community Division as part of Walsall's continued progress towards integrated working with our local partners. It is an opportunity to work within a Trust which has advanced levels of integrated working and a commitment to maintaining excellence within our Community Teams.
The Division has recently undertaken a review of the service and has a plan to ensure that the needs of the families of Walsall continue to be met as well as ensuring that the Service can evolve by developing and supporting all of its staff to meet these needs.
Main duties of the job
As a staff nurse in the Health Visiting service you will work within a skilled mix team and contribute to the development, implementation and evaluation of interventions within the Healthy Child Programme 0-5. This will include individual and group programmes for children and parents/carers and training for staff in early years and partner agencies. You will manage your own caseload of children to assess, plan, deliver and evaluate care. Your work will be delegated by a Health Visitor

Job responsibilities
- Responsibility for planning, co-coordinating and reviewing own caseload in conjunction with the team. Ensuring that workload is appropriately prioritised and managed. This includes children who have moved into the area, have medical needs, faltering growth, recall due to developmental needs and children on plans.
- Carry out allocated mandated checks (part of HCP 0-5) as delegated by a Health Visitor e.g., 9-12 months developmental check, 2-2.5 year review.
- Undertake holistic health assessments and plan, implement and evaluate evidence based nursing care for individual children and their families.
- Support the development, planning, implementation and evaluation of health education and promotion packages using evidence based materials and methods.
- Where appropriate, contribute to multi agency processes to assess and develop care plans to support children health and wellbeing e.g. children who are Looked After, in need of safeguarding, in receipt of Early Help.
- Have a good knowledge of local services in health and partner agencies and where appropriate, liaise and refer a child or their parent/carer to multi-disciplinary teams and other specialist services.
- Where appropriate, take on the role of lead professional to coordinate delivery of care plans for children e.g. Children on Early Help plans
- Working in partnership with the multi-agency team provide clinical advice, information and support regarding children with long term conditions, disability or specific health problems to minimise negative impacts on their lives and promote health gain and social inclusion to the children, family and carers.
- Support the development, planning, implementation and evaluation of health promoting activities with individuals and groups, utilising evidence-based materials and methods.
