Job responsibilities

As a staff nurse in the HCP 0-19 Sendi team you will work within a skilled mix team and contribute to the development, implementation and evaluation of interventions within the Healthy Child Programme 0-19 (24 years old for CYP with additional needs). This will include individual and group programmes for children, young people and parents/carers and training for staff in education and partner agencies. You will manage your own caseload of children and young people to assess, plan, deliver and evaluate care.

Support the development, planning, implementation and evaluation of health education and promotion packages using evidence-based materials and methods.

Where appropriate, contribute to multi agency processes to assess and develop care plans to support children and young peoples health and wellbeing e.g. children who are Looked After, in need of safeguarding, in receipt of Early Help.

Have a good knowledge of local services in health and partner agencies and where appropriate, liaise and refer a child or young person to multi-disciplinary teams and other specialist services.

Where appropriate, take on the role of lead professional to coordinate delivery of care plans for children and young people e.g. Children on Early Help plans

Working in partnership with the multi-agency team provide clinical advice, information and support regarding children with long term conditions, disability or specific health problems to minimise negative impacts on their lives and promote health gain and social inclusion to the children, family and carers.

Support the development, planning, implementation and evaluation of health promoting activities with individuals and groups, utilising evidence-based materials and methods e.g. SRE, EHWB, Height Weight

Develop and maintain effective communication networks and work collaboratively with key partners, health workers, statutory and voluntary agencies to actively promote national and local public health priorities.

Provide mentorship to learners including, pre and post registration nurse and mentorship, supervision and support for members of the team as directed by Clinical Team Leader, commensurate with knowledge and skills.

• RN,RSCN, Child Branch or Mental Health Registered Nurse
• Demonstable evidence of professional development

• Able to carry out assessments through appropriate. questioning, observations, and accurate recording
• Able to collate and interpret information to devise. intervention based on formulation
• Able to deliver effective clinical intervention with children, and their families.

Knowledge

Desirable

• Knowledge of health child programme
• knowledge of child safeguarding practices and policy
• Knowledge of children with additional needs and disabilities
• knowledge of public health national and local agendas
