It is an exciting time to join the HCP 0-19 Service within Walsall Healthcare NHS Trust. The service has recently been transferred into the Community Division as part of Walsall's continued progress towards integrated working with our local partners. It is an opportunity to work within a Trust which has advanced levels of integrated working and a commitment to maintaining excellence within our Community Teams.
The Division has recently undertaken a review of the service and has a plan to ensure that the needs of the families of Walsall continue to be met as well as ensuring that the Service can evolve by developing and supporting all of its staff to meet these needs.
Main duties of the job
As a staff nurse in the HCP 0-19 Sendi team you will work within a skilled mix team and contribute to the development, implementation and evaluation of interventions within the Healthy Child Programme 0-19 (24 years old for CYP with additional needs). This will include individual and group programmes for children, young people and parents/carers and training for staff in education and partner agencies. You will manage your own caseload of children and young people to assess, plan, deliver and evaluate care.
