Intermediate Care Services Team

An exciting opportunity has arisen for a qualified mental health nurse to join the Intermediate Care Services (ICS) team at Walsall Healthcare NHS Trust. The post holder will form part of a multi-agency team supporting the hospital to achieve safe, timely and effective discharges from the ward areas. The post holder will be expected to work within the multi-disciplinary team to provide senior mental health nurse advice and expertise in relation to complex discharge planning. They will be responsible for completing comprehensive, holistic assessments and care plans to facilitate safe and timely discharge. The post holder will also support junior members of the team to promote quality and embed the ethos of the ICS.

Main duties of the job

The post holder will be expected to complete patient visits in the community post discharge in both patient homes and nursing and residential placements to plan for long term care and support. The successful candidate will require baseline knowledge of Continuing Healthcare national framework as assessing for eligibility of such funding is a main requisite of the role.

Applicants should have at least 2 years post registration experience preferably on an acute ward and have experience and knowledge in discharging complex patients and CHC funding and pathways. A knowledge of social services is also desirable to the post. Applicants must hold a registered mental health nurse qualification. Candidates must be able to travel in the borough and surrounding areas and have access to a car for this purpose.
