Job summary

This is an exciting opportunity for an experienced Senior Staff Nurse who has Paediatric community experience of working with CYP with additional needs and disabilities. You will work with our newly formed team which support babies, children and young people with additional needs and disabilities. The team is part of our Healthy Child Programme Service 0-19 which incorporates Health Visiting, School Nursing and Teenage Pregnancy Services. It is an exciting time to join our service and be part of developing and shaping services for babies, children, young people and their families.

This exciting opportunity has arisen for a dynamic, motivated and compassionate qualified Senior Staff Nurse to join our newly formed team which reflects the need to be increase capacity to provide care and support babies, children and young people with additional needs, disability and/or SEN.

You will be part of wider team around the child provision which is well established for 0-5 and has an international reputation, recently having presented at good practice conference in Europe.

The ideal candidate with have significant experience working as a qualified Senior Staff Nurse and working with babies, children and young people with additional needs, disability and/or SEN. You will have an excellent record of working in partnership with other agencies and services

Main duties of the job

To manage a caseload of children and families with complex needs undertaking appropriate assessment, intervention and evaluation through programmes of care and support to meet identified needs.

To provide a consultancy and advisory role in respect of childhood disability issues, especially to mainstream services and in particular Children's centres.

To make recommendations for innovations and service wide changes in practice and be responsible for the implementation of such change

To undertake direct holistic assessment of highly complex disabled childrens / young peoples needs in the context of their family and environment. Following standards of TAC initial assessment. To carry out the assessment, planning, implementation to most effectively meet the needs of the child and family.To provide support, intervention and evaluation through programmes of care which are based on evidence to meet identified needs and to deliver programmes of care in appropriate localities to include childrens centres, domiciliary, nursery and clinic. Work collaboratively with other disciplines and agencies across Health, Social Services and Education to ensure that services for individual families are co-ordinated, seamless, timely and robust To act as Initial Co-ordinator and Key Worker for delegated children and families following the standards and processes as outlined in the TAC Key Worker handbook in particular linked to complex cases including child concern, safeguarding and social issues Work in partnership with parents and carers to enable and empower families of disabled children, to function effectively, access appropriate services and anticipate and respond to their childs changing needs and also influence service delivery To communicate highly complex and sensitive condition related information to parents and carers to:

    • Promote their understanding of the childs condition and its possible outcomes
    • Initiate adjustment to it
    • Enable parents/carers to make informed decision

To allocate work to and supervise the work of Community Nursery nurses and contribute to the development of their skills and knowledge
