Job summary
- To work as part of an integrated Community nursing team with a focus on case management for adults with bladder and/or bowel dysfunction.
- To deliver specialist skills and expert knowledge to assess the physical and psycho - social needs of a defined client group with bladder bowel and/or continence dysfunction, Instigating therapeutic treatments, including extended and supplementary prescribing, based on best available evidence in order to improve health outcomes.
Main duties of the job
To deliver specialist skills and expert knowledge to make a comprehensive and focused assessment.
Advise on a range of management options to support plans of care e.g. provision of containment products, appliances and devices to patients, carers and other health care professionals.
Work in partnership with GPs, consultants in Secondary care, Integrated community nursing team to ascertain diagnosis, develop care plans, interpret highly complex factors and situations and initiate follow up for people who have bladder, bowel and continence dysfunction. .
Negotiate and agree with Patient, Carers and other care professionals, individual roles and responsibilities with regard to actions to be taken and outcomes to be achieved, referring on to other services or professionals as appropriate.
