Job summary
We are seeking a highly motivated, enthusiastic and clinically skilled nurse to join our dedicated, friendly Palliative & End of Life Care team, as an Associate Clinical Nurse Specialist, based at Dorset County Hospital NHSFT. The post holder will be expected to deliver a high degree of professional autonomy, make clinical judgements using advanced knowledge and skills in order to provide the highest standard of holistic patient care, developing and practicing within clinical protocols and guidelines.
Supported by the Band 7 team, you will be expected to manage a small, changing caseload of patients with complex needs, as well as contributing to our education programme and the ongoing development of the palliative and end of life care service.
Main duties of the job
- Supported by a Band 7 CNS, manage a caseload of patients and their families/carers with specialist palliative care needs, assessing, developing and implementing a plan of care.
- Establish and maintain highly effective communication with patients, carers, and professionals across health and social care to develop therapeutic relationships.
- Act as an advocate for the patients, families and carers.
- To utilise palliative knowledge to educate generalist colleagues to develop their competence in fundamental symptom control.
- Providing holistic assessment of needs and implementation of patient centred care.
- Work closely with the multi-professional team and to support them in providing clinical recommendations to ensure optimal palliative care is delivered.
- To work closely with site specific cancer CNS's, the Acute Oncology
- Team and other chronic disease CNS's to manage people's needs.
- Respond to referrals across the whole of DCH including outpatients and the emergency department.
- Liaise with community palliative care teams and other community-based health and social care providers to promote seamless care of people with palliative care needs.
- Demonstrate a fundamental level of specialist knowledge in relation to specialist palliative care, including end of life care and keeping up-to-date with all relevant developments in these areas.
