An autonomous practitioner, alongside the Paediatric Diabetes team, including clinical lead and medical colleagues, responsible for the assessment, primary and/or secondary diagnosis, management and evaluation of treatment and care for a caseload of children & young people aged 0-18 years with diabetes in both the hospital and community.

The post holder will provide nursing care for a defined caseload, educating and supporting the child/young person and their family/carers in diabetes self-management skills from diagnosis to transition.

Co-ordinate and implement an overall strategy of evidence-based care for the whole caseload of patients

Co-ordinate the service alongside the Clinical Lead and lead practitioners for medicine, nursing, dietetics and psychology.

Be a highly visible and accessible resource for specialist advice to professionals and carers providing care to children and young people with diabetes.

Through effective leadership, ensure the clinical multidisciplinary team deliver a safe, effective, responsive, caring and well-led service where compassion is embedded, and a positive patient experience is achieved.

Provide strong professional clinical and managerial leadership and act as a professional role model.

Works across organisational and professional boundaries, including working collaboratively with health care professionals in the community and external agencies, for example social care, education and the voluntary sector.

Act as an expert resource in Paediatric Diabetes with extensive specialist knowledge and clinical expertise to be an effective educator for health care professionals, CYP, their families and carers (including the education setting).

Operate as Nursing Lead for Education overseen by the Nursing Leads. This incorporates the education requirements for ward staff, schools and patient education programmes.

Work collaboratively with the Clinical Lead and service leads (e.g. medicine, dietetics and psychology to deliver the service to national standards such as the RCPCH National Diabetes Quality programme currently PEER Review and BPT

As a nurse prescriber (willing to undertake) demonstrate advanced knowledge of and experience in the use of Oral Hypoglycaemic Agents (OHAs), insulin and other treatment regimens relating to diabetes.

As a nurse prescriber advice on the stabilisation of patients on all oral agents, insulin, and other injected therapies according to national, regional, and locally formulated guidelines and protocols.

As a nurse prescriber demonstrate advanced knowledge and experience in prescribing treatment within your scope of practice alongside local policy.

You should possess the ability to perform assessments of patient needs and liaise with other members of the multi-disciplinary team where necessary.

Person Specification

Qualification & Experience

Essential

• Registered Child branch or Registered Sick Children's Nurse (RSCN),
• Educated to BSc Degree level in relevant field
• Recognised Post graduate Paediatric Diabetes Qualification
• Leadership development programme or willingness to undertake leadership training
• Independent Non-Medical Prescribing qualification (Active on the NMC register) or willingness to undertake

Experience and Knowledge

Essential

• Significant demonstrable experience within paediatric diabetes care with evidence of professional development within this specialism.
• Minimum 5 years post registration
• Evidence of developing policy, guidelines and managing resources

Desirable

• Previous experience in line managing staff
• Teaching qualification or equivalent
• Experience of working collaboratively with a range of other services within primary, secondary and tertiary care, social care and education

• Direct care skills in hospital and community settings (i.e. Insulin dose adjustment, able to run pump and CGM starts, history taking, assessment, treatment and evaluation in relation to diabetes)
• Ability to make autonomous clinical decisions and judgments

• Coaching and mentoring skills
• Critical appraisal skills
• IT literate
• Ability to gather data, compile information and write reports
• Knowledge and understanding of national paediatric diabetes guidelines

• Car owner with valid driving licence

• Advanced Communication Skills Training
• Understanding of health policy and the national and local nursing agenda
• Prior experience of conducting nurse-led clinics independently
