Job summary

  • The post holder will provide assistance to the qualified nurse in creating and developing a stimulating activity programme that enhances the rehabilitation, quality of life and mental well being for each patient, promoting a shorter length of stay for the patient in a hospital environment.
  • They will assist in planning and coordinating an activities programme that promotes active daily living for each patient as an individual or within a group session.
  • They will provide activities that have identified the needs of the collective group and/or individual through a collaborative approach that encourages the involvement of patients and their carers/family.
  • They will work as part of a nursing/multidisciplinary team to action any assistance required to deliver fundamental care needs as they arise.They will work within Trust policies, procedures and guidelines
  • They will contribute to the cleanliness and tidiness of the ward environment
  • To contribute towards the smooth running and development of the department through effective teamwork.

Main duties of the job

The post holder will have previous experience workingwith the older person withDementia and/or Orthopaediccomplaints. They are to be precise and methodical in their working practice and able towork under own initiative. The post holder is required to have1 year experience working as ahealth care/therapy assistant in acare environment.

The post holder must have excellent communication skills and the ability to work in a calm and patient manner.

To work with the multi-disciplinary and wider hospital and volunteer teams to provide an activity programme for individual or groups of patients either in the day room or at the bed side.

About us

  • Purbeck Ward consists of 27 beds
  • The ward treats predominantly emergency Orthopaedic and Trauma patients, however at times depending on the capacity of the wider Trust will also cater for patients outlying from other specialities.
  • Purbeck Ward is the Orthopaedic and Trauma ward at Dorset County Hospital, which in turn is a Major Trauma Unit as part of the Wessex Trauma network.
  • The Day Room feature has been in place since 2018.

Purbeck ward is a very busy but friendly ward, with the emphasis on working together for the good of our patients.

Job description

Job responsibilities

  • Assist with the care of the patient's physical, mental, psychological and personal needs, ensuring a high standard of care is achieved in a kind and caring manner.
  • Deliver a daily activity programme that promotes rehabilitation, prevents deconditioning and provides cognitive stimulation by utilising various different therapeutic methods on a daily basis to ensure that the diverse needs of a patient requiring rehabilitation are met.
  • Assist and motivate the patient to participate in group/individual activity sessions to promote and stimulate physical and mental well-being through interaction with others, ensuring it is safe to do so by liaising with the MDT.
  • Provide individually tailored programmes to patients with specific requirements.
  • Engage the MDT in the decision making process where an activity programme is required to be adjusted in order for it to meet an individuals special requirements.
  • Utilise the This is me form to promote a person centred interaction with the patient and encourage all members of the MDT to utilise this method of communication.
  • Provide kind and sensitive support for the patient and carers/relatives.
  • Utilise best practice in communication skills that promotes positive interactions with theolder person who has dementia.
  • Demonstrates a personal commitment to challenging any poor practices from any members of the MDT with regards to the provision of good standards in Dementia Care.
  • Encourage independence and promote personal choice decisions, whenever possible, for the patient. Take advice from the nursing staff regarding any concerns or highlighted safety issues that may impact on the patients well being.
  • Provide individual patients one to one care and the time to complete tasks in an unrushed manner to promote and encourage independence as required.
  • Assist patients to mobilise safely, including encouragement to use the correct aids available.
  • Work with the MDT to assist the patient to participate in the therapeutic rehabilitation treatment plan to ensure rehab is 24/7.
  • Promote and support communal dining in order to improve nutritional intake, ensuring good hygiene practice is adhered to at meal times and any adaptive cutlery/crockery is used to aid independence in feeding when required.
  • Chaperone patients who require support whilst undergoing medical examinations/procedures.
  • Assist with patient flow from time to time, safely transferring patient to other areas.
  • Ensures that the day room/activity centre is arranged to promote a homely feel for the patients and their carers/family, removing all unnecessary clinical items from that area.
  • Maintain a clean and safe environment
  • Be compliant with all the guidelines in the prevention and control of infection policies and procedures at all times within the clinical area, in order to ensure the highest quality of care to patients.
