An exciting opportunity has arisen for a Band 7 Parkinson's Nurse Specialist to join our team here at Dorset County Hospital NHS Foundation Trust.Ideally the candidate will already be working as Parkinson's disease Clinical Nurse specialist Band 7 , however we will consider candidates with a keen interest who wished to develop into this role at a Band 6 with a view to Band 7 on completion of training and competencies - this will include a Non-Medical Prescribing qualification.There will be protected time for education and training and an expectation that you will undertake the Non-Medical prescribing programme, if you do not have this in place. We will use the competency framework to tailor future study in collaboration with Parkinson's UK.In the role of Parkinson's nurse specialist, you will manage a specific patient group through the whole patient journey. This will include providing specialist advice to patients, carers/relatives, clinical practitioners and peers in a variety of settings such as our community, hospital (in-patient/out-patient) and residential and nursing homes.You will work collaboratively and co-operatively with clinical colleagues across the Trust and within the community to develop services, focused on patients being at the centre of our care.
Main duties of the job
Providing an equitable and seamless service for people with Parkinson's Disease (PD) throughout all stages of the disease.
- To take responsibility for managing practice, using an advanced level of professional accountability, autonomy and judgement for complex decision-making for patients with a diagnosis of Parkinson's and Parkinson's Plus syndromes through all stages of their condition from newly diagnosed through to palliative stage.
- To work autonomously, using advanced knowledge and skills, underpinned by theory and experience, to provide specialist expertise in planning, implementing and monitoring care, facilitating evidence-based, patient-centred care for patients with Parkinson's or Parkinson's Plus syndromes.
- To plan and manage complete episodes of care, working in partnership with others and, delegating and referring as appropriate, to optimise health outcomes and resource use, as well as providing direct support to patients and carers.
- To facilitate patient empowerment by enabling patients with Parkinson's to manage their health and achieve better outcomes.
- To act as a resource to others and to the delivery and evaluation of a range of relevant specialist programmes of education and development for individuals and large groups.
- To monitor and develop the Parkinson's service across DCH to ensure that the service is effectively and efficiently managed in the delivery of a high quality service.
About us
Our Parkinson's Team is a small team comprising of 2 specialist Nurses (including this role) and a clinical lead, with close links to the Neurology Team, hosted by University Hospitals Dorset NHS Foundation Trust. The team also works closely with the community team hosted by Dorset Healthcare NHS Foundation trust.
Our community services for adults are provided in people's homes, our main hospital site, Community Hospital site(s) and Nursing / residential homes. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
