Job summary

We are looking for an enthusiastic and motivated Nurse or Allied Health Professional wanting to take on new challenges in order to deliver the best outcomes to our patients. This includes ensuring timely access to a range of community health and social care services for patients leaving the hospital working within a discharge to assess model.

Main duties of the job

Are you. . .

  • Are you an experienced clinician with drive and enthusiasm?
  • Are you able to manage and prioritise your own caseload?
  • Are you an exceptional, friendly and dedicated healthcare professional?
  • Do you have experience in the discharge to assess model?
  • Do you have an interest or experience in complex discharges including MCA assessments?
  • Are you able to work with a wide range of patients of varying age groups and conditions?
  • Are you able to communicate complex processes clearly and with compassion?

About us

Do You have. . .

  • An NMC or other AHP registration (e.g. HCPC)
  • Experience or interest in complex discharges
  • Good computer skills and some experience of databases.
  • Experience in teaching for mentoring staff

EDUCATION, QUALIFICATIONS & TRAINING

Essential

  • Registered Nurse or equivalent health care professional

Desirable

  • Evidence of CPD
  • Ability to present and teach

KNOWLEDGE & EXPERIENCE

Essential

  • Minimum of three years relevant post registration experience including working with Primary and secondary care and adult services
  • Recent and significant experience of working with patients / carers/ MDT in preparing for discharge from hospital
  • Experience of producing and assimilating report using a variety of data systems.
  • Recent experience of working across primary and secondary care
  • Specialist knowledge of the discharge to assess model and discharge pathways
  • Knowledge of the Mental Capacity Act, Safeguarding Adults and Deprivation of Liberty.

SKILLS & ABILITIES

  • Evidence of an ability to maintain effective working relationships
  • Evidence to implement and facilitate change at departmental and organisational level.

Desirable

  • Demonstration of excellent written and verbal communication skills across all levels.
  • The ability to work flexibly in order to meet the service / organisation requirements.
