Pressure Ulcer Prevention Nurse

Job summary

We require a pressure ulcer prevention nurse who will work 3 days a week (0.6WTE). The main base for the post holder will be Teddington Health and Social Care Centre. The job will include:

To have an overview of the prevalence and incidence of pressure ulcers across KRFT and ensure the key messages are conveyed to senior operational and clinical managers regarding identified need for change, compliance requirements and local needs of services in relation to pressure ulcer prevention and management.

work in partnership with clinical colleagues providing care in the community and in-patient unit to deliver high quality pressure ulcer prevention and management.

To provide professional and clinical leadership and expertise in pressure ulcer prevention and act as an expert resource for education and clinical expertise in the delivery of pressure prevention care interventions in the community.

To support the provision of services to patients at risk from and with pressure ulcers which includes risk assessment from admission to discharge and participating in joint assessments and patient reviews.

To be competent and up to date in treatment options for patients requiring pressure ulcer care.

Main duties of the job

Work with teams across KRFT to improve their knowledge and skill in pressure ulcer prevention and management.

Work in partnership with the team/service managers and others to embed clinical improvement in practice.

Provide specialist, high quality, highly complex and expert nursing care and support for patients and carers in relation to all aspects of pressure ulcer development and management.

Act as an expert resource for patients and the clinical team by the provision of comprehensive advice (written and verbal), information, teaching, training, and clinical role modelling with patients

Undertake where appropriate, nursing research with published outcomes and updating own knowledge to promote excellence in clinical practice and the setting of quality standards within tissue viability and KRFT.

Lead and/or undertake clinical audits and quality improvement benchmarking audits across clinical services, undertaking the analysis of findings and identifying learning to contribute to the development of action plans with teams.

Deliver the pressure ulcer prevention improvement plan in the community.

  • First Level Registration with NMC valid registration.
  • BSc in Tissue Viability or related subject or evidence of working towards
  • Clinical Expertise in tissue viability supported by extensive education and professional development to 1st degree level.
  • Mentorship/Preceptorship or equivalent teaching qualification.

Experience

Essential

  • A practitioner with community or acute Tissue Viability or acute or community related speciality
  • Experience in managing wounds
  • Experience in pressure ulcer prevention and management practice
  • Experience of MDT working

  • Knowledge of current advances in wound care, pressure ulcer management
  • Excellent interpersonal and communication skills
