Job summary
You will act as a Keyworker for metastatic breast cancer patients providing a link between patients and health care professionals across primary, secondary and tertiary care and act as an advocate for patients/families.
Kingston and Richmond Hospital NHS Foundation Trust (KRFT) is pleased to be able to work in partnership with the Royal Marsden Hospital Medical Oncologists in the provision of breast cancer treatments.
You will ensure your role reflects the core components of the clinical nurse specialist which includes clinical practice, management/organisation, education, research and audit, taking into account the national cancer agenda and its impact on service delivery.
You will continually develop, implement and evaluate the Metastatic Breast Cancer Service to improve and maintain standards of patient care and promote a seamless service within a multi-professional framework.You will be a clinical expert in breast cancer with excellent communication skills. The role requires the ability to work autonomously and independently but within an integrated collaborative team.
KRFT breast cancer nursing team is an active member of the Pan-London cancer services network, and the post-holder will be expected to participate in the Pan-London Breast Care Nursing Group.
You will need to work on a full day on a Wednesday then either a Monday or a Thursday.
Main duties of the job
- You will work alongside the medical oncologists and collaboratively with the acute oncology service (AOS). You will refer to the specialist palliative care services where appropriate. Promoting a seamless pathway of care and prevent crisis, inappropriate admissions and reduce hospital inpatient stay.
- Develop and facilitate in Holistic Needs Assessment (HNA) clinics for patients at the point of recurrence or diagnosis of secondary disease, during treatment and at any point of when individual needs arise thereafter.
- Provide a physical /virtual presence and point of contact for patients with metastatic disease either in person or by telephone contact.
