Kingston & Richmond NHS Foundation Trust is excited to be recruiting five Consultants in Geriatric Medicine to help shape the future of frailty care across south west London. We are at the forefront of frailty service transformation, with investment to expand our Frailty Same Day Emergency Care (SDEC) unit and community frailty pathways.
Our Trust has a proven track record of innovation in frailty and is proud to be one of only seven systems nationally selected to join the NHS England Frailty Improvement Collaborative. We are also leading partners in the London-wide frailty network, co-designing integrated, patient-centred approaches that reduce avoidable hospital admissions and deliver more care closer to home.

Working across our acute frailty unit, Care of the Elderly wards, integrated Virtual Ward, and community rapid response services, you will play a central role in designing and delivering new models of care that cross organisational boundaries.

Main duties of the job
Post-holders will deliver high-quality geriatric medicine across acute, community, and frailty pathways. The new model of care offers rotational roles spanning:
Consultant-led ward care on dedicated Care of the Elderly wards.
Front-door Frailty SDEC service, including early supported discharge , admissions avoidance
Integrated community frailty care via the Virtual Ward and rapid response MDTs
Opportunities to develop specialist liaison roles (surgical, orthogeriatric, or with other specialties)
Consultants will be expected to provide clinical leadership, contribute to shaping new frailty pathways, work collaboratively with colleagues across primary, secondary, and community services.
Successful applicants will also play an active role in teaching, training, audit, and service improvement, supported by the Trust's strong quality improvement programme.
With the significant investment into frailty services, this is a chance to help build a seven-day integrated model of care that genuinely improves outcomes for older people. Flexible and less-than-full-time working arrangements are welcome.
