Palliative Care Clinical Nurse Specialist

Job summary

We are seeking a compassionate and skilled Specialist Palliative Care Nurse to join our team in delivering high-quality, patient-centered care. This advisory role involves supporting patients with cancer and non-malignant conditions, as well as their families, by providing expert guidance on symptom management, psychosocial support, and end-of-life care.

As a key member of the multidisciplinary team, you will act as a resource for clinical colleagues across the Trust, offering specialist advice on palliative care issues. You will also play a vital role in education and training, facilitating learning for staff at all levels and contributing to the development of evidence-based palliative care programmes.

This is a 7-day service, and the successful candidate will participate in a face-to-face visiting rota operating from 9am to 5pm.

Main duties of the job

Clinical Excellence

  • Places quality at the heart of practice by delivering evidence based individualised care, through holistic needs assessment, planning and evaluation of care.
  • Acts as a Keyworker for specialist palliative care patients.
  • Facilitates highly sensitive discussions and ethical decision making around end of life issues.
  • Provides clinical expertise, specialist advice and support across service boundaries including the medical, allied health professional and other members of the multidisciplinary teams involved in caring for patients with palliative care needs.
  • Liaises with Cancer Services, Community Palliative Care teams and local Hospices, GPs and other community health professionals

Innovation:

  • Works across professional boundaries using creative reasoning and problem-solving to facilitate new ways of working to influence improvements in the patient care pathway.
  • Instigates and manages change within a complex environment. Is able to evaluate the impact of change and any impact on the patient care pathway.
  • Promotes patient experience feedback through local initiatives to drive innovation and change.

Leadership:

  • Acts as a role model demonstrating high standards of care and providing clinical leadership to others.
  • Provides specialist education and training to other professionals involved in patient care.
  • Leads the development, management and promotion of the specialist nursing service ensuring the delivery of high quality, cost-effective care.
