Job summary

To lead, deliver, develop and evaluate clinical pharmacy services within the Wolverton Centre for Sexual Health and HIV in accordance with the objectives set by the Clinical Director Sexual Health & HIV and the Senior Principal Pharmacist

To support the Clinical Director for HIV/Sexual Health in the delivery, development, evaluation and co-ordination of clinical HIV services in line with the objectives of the specialty and the Trust

To be responsible for the daily management of the HIV clinical service reporting to the Clinical Director and consultant lead for HIV

To develop, organise, provide and evaluate pharmacy services to HIV patients to ensure that drug use is safe, efficient and cost effective.

To optimise pharmaceutical care for HIV patients attending the HIV service including regular medicine reviews

To provide adherence support to all adult patients considering, starting, taking or changing antiretroviral (ARV) therapy who attend the Wolverton Centre, according to individual need and local and national guidelines.

To be the pharmacy lead for clinical trials in Sexual Health & HIV

To provide training in HIV & sexual health for pharmacy staff in the hospital

To provide HIV medicines advice and reviews for HIV inpatients

To develop and maintain the Wolverton new electronic prescribing and documentation system (Inform)

Main duties of the job

1. To be responsible for the delivery and management of all pharmacy services at the Wolverton Centre for Sexual Health and HIV.

2. To identify those patients who require increased support and liaise with other pharmacists within the Trust, members of the multidisciplinary team and with Secondary/Primary Care, as appropriate.

3. To demonstrate clinical reasoning and judgement.

4. To demonstrate professional accountability to patients and their carers.

5. To liaise with other clinical pharmacists to ensure efficient transfer of medicine related care.

6. To accurately dispense, check medicines and counsel patients and/or their carers regarding their medication as required.

7. As a prescriber, prescribe medications for HIV/sexual health patients at Kingston Hospital within the remit of the registered scope of practice and drug list and also in accordance with the non-medical prescribing policy.

8. To regularly review the clinical pharmacy service at the Wolverton Centre and ensure it meets local and national guidelines.

9. To work closely and communicate effectively with the multi-disciplinary team to ensure appropriate co-ordination of the pharmaceutical service with patient care.

10. To be Responsible for the co-ordination of adverse drug reaction (ADR) reporting from within the HIV/ sexual health specialty.
