Job summary

Are you a compassionate and skilled healthcare professional passionate about making a difference in patients' lives at the end of life? Kingston Hospital Foundation NHS Trust seeks an experienced Community Nurse to join our dedicated team. This role will focus on delivering exceptional end-of-life care for frailty patients across community and secondary care settings. You will play a crucial role in ensuring patients receive care aligned with their preferences, promoting dignity and comfort in their final weeks and months.

Main duties of the job

Key Responsibilities:

  • Integrated Care: Work across community and secondary care sites to advance the end-of-life care agenda for Kingston and Richmond, ensuring seamless care delivery for patients with frailty and non-malignant diseases.
  • Patient-Centred Service: Facilitate a wraparound service to enable patients to be cared for and die in their preferred place, respecting their wishes and needs.
  • Advance Care Planning: Collaborate with healthcare partners to complete advance care planning, minimizing unnecessary hospital time and promoting personalized care.
  • Local Partnerships: Build robust links within the health and care community to ensure continuity and quality of care for patients across all agencies.
  • Clinical Expertise: Provide high-quality generalist palliative care, offering symptom control advice, information, and psychosocial support. Act as a resource for other professionals on palliative and end-of-life care issues.
  • Leadership and Improvement: Lead quality improvement projects with Nurse and Medical Consultants to enhance care services and outcomes
