Are you passionate about delivering exceptional patient care? We have an exciting opportunity for an Advanced Clinical Practitioner (ACP) to join our Unplanned Care Division in the Medical Unit and Acute Assessment Unit (AAU).

In this vital role, you'll play a pivotal part on our medical wards, working across all areas of the Medical, Care of the Elderly and AAU wards on a rotational basis while delivering expert clinical care and leadership. You'll be at the forefront of patient-centred practice, using your advanced skills and knowledge to make independent decisions, initiate interventions, and manage complex cases. Your leadership extends beyond direct patient care, as you'll guide and support nurses, colleagues, and junior staff, contributing to a thriving team environment.

Main duties of the job

As an Advanced Clinical Practitioner, you'll wear many hats. Clinically, you'll be a leader, providing compassionate, evidence-based care, performing examinations, and formulating diagnoses. Beyond direct patient care, you'll act as a trusted advisor, championing best practices, driving service improvements, and fostering teamwork. You'll prioritise patient well-being through individualised attention, informed consent, and safety advocacy. You'll drive efficiency through innovative initiatives, quality improvement, and collaboration with colleagues. Finally, you'll be a dedicated mentor, nurturing talent and supporting both trainee and experienced team members. All while upholding the highest professional standards and actively pursuing your own development. This is a chance to make a real difference, both for patients and the healthcare team.

  • The development of ACP roles in medicine in KHFT will both support the diversification of the medical workforce, but also support specialists to develop generalist approaches to address the growing number of people with multi-morbidity.
  • We have a strong history of the value and joint working of ACPs in other areas of the Trust (namely SDEC & ED)
  • The integration of ACPs in medicine is not only describing how we envisage ACPs working closely in the multidisciplinary ward teams, but also developing relationships with the patients' carers, volunteers and potentially community services.
