If you are a dynamic Nurse with experience in heart failure, keen to work in our Community Heart Failure Service in Richmond Community, we'd love to hear from you.

Richmond's Community Heart Failure Service has been developed to meet the ever-increasing needs of our ageing population. We are looking to recruit an innovative and dedicated Community Heart Failure Nurse Specialist, to continue to develop our excellent service. This would involve working closely with GP Partners and Consultant Cardiologists. The post holder will provide a community heart failure service which will encompass home visits, community-based clinics and supporting heart failure rehabilitation classes. The successful applicant will be expected to devise individualised care plans for our patients with heart failure and working closely with our Community teams to educate and support in the management of heart failure.

You will be expected to work autonomously and will be part of a wider team of cardiac specialist nurses, currently leading services across the cardiac spectrum at Kingston Hospital and in the Community.

Main duties of the job

To provide high-quality, patient centred, community heart failure care.

In conjunction with the KHFT Heart Failure Nursing Team and the Heart Failure Consultants, the post holder will work autonomously as an advanced practitioner managing patients in the community with heart failure. (This is a lone worker role, with remote support from the Cardiology team at Kingston Hospital).

The post holder will provide high level clinical assessment, care planning, expert advice, extended intervention and thorough evaluation.

The post holder will work collaboratively with Primary and Secondary care teams to optimise the management of patients with heart failure, providing advice to primary and community colleagues, assist with the reduction of inappropriate referrals and unplanned admissions and ensure that patients with heart failure receive holistic and integrated care.
