Job summary
We are offering an exciting opportunity for an experienced Clinical Educator to join our well established and dynamic hospital palliative care team.
You will be committed to training and educating health care staff across the Trust on care of the dying patient and all aspects of end of life care. You will also contribute to the teams' development, research and governance through the processes of audit and bench marking, and help to drive forward palliative education and service development initiatives at the Trust.
Main duties of the job
To support and deliver palliative and end of life care education and training to health care staff, and to further develop evidence based palliative care educational programs at Kingston Hospital
To take primary responsibility for providing a progressive learning environment for all grades of nursing / care staff and support the development of less experienced palliative care nurses.
To take a lead role in the education of care of the dying patient within the Trust
To take a lead role in the Trust for Advance Care Planning (ACP) including promoting the use and uptake of ACP systems, with support from the Lead Physician and Senior CNS. This involves working with clinical teams to promote early identification of patients thought to be in the last months of life, encouraging and supporting discussions around the patients and family's wishes and preferences for treatment and care and documenting/communicating these preferences
To be actively involved in project management and service development using quality improvement methodology.
