This is an exciting opportunity to join a progressive innovative Diabetes and Endocrinology team.
We are looking for a Diabetes and Endocrine HCA to work in the Diabetes Day Unit and Endocrine department outpatients, providing both clinical and administrative support to the team.
You will be a highly motivated individual with a confident and professional attitude. This post will develop within, to meet the needs of the diabetes/ endocrine service and the post will evolve over time. Flexibility and a willingness to work in different roles in the service are desirable and encouraged.
The Diabetes Day and Endocrine Unit is committed to the development and support of its staff.
You will have the full support of other Adult Diabetes Specialist Nurses, Dietitians, Registrars and Consultants as well as the leadership and support provided by the Nurse Consultant

Main duties of the job
The postholder will work as part of the Diabetes and Endocrine team working alongside registered nurses, medical staff and allied healthcare professionals who provide support, advice and treatment to people with diabetes and endocrine disorders through the provision of education and advice on aspects of diabetes/endocrine management.
To be part of the diabetes/endocrine outpatient team to assist with the smooth running of clinics and patient flow through the appointment pathway.
To undertake HCA duties specific to the diabetes/endocrine outpatients service

Job responsibilities
KEY RESPONSIBILITIES/OUTCOMES:
- To be responsible for ensuring that any patient not currently listed on the system is registered and that their demographic details are current and accurate.
- To ensure that transport and interpreters are booked where necessary.
- To prioritise the daily workload as per speciality to ensure that any referrals received, and all urgent matters are dealt with as soon as practicable. To escalate when capacity is an issue and when work needs prioritising.
- To action requests for appointments and follow up as specified on a patients discharge summary in line with Trust policies.
- To cover colleagues during absence and ensure patient registration is not delayed and where patients need to be booked appointments to avoid breaches are prioritised.
- Communicate clearly, effectively and appropriately with the multi-disciplinary team
- To order blood tests as directed by the responsible clinician
Clinical
- To assist the clinicians in the smooth running of Diabetes outpatient services (or work under the indirect and direct supervision of a registered nurse/practitioner)
- To undertake basic vital signs such as blood Pressure, weight and urine check for outpatients
- Act as a chaperone as required
- To carry out a full foot check specific to the needs of a diabetes patient
- Support and promote health education and or teaching advice, practical skills and diabetes knowledge to patients, their families and carers including the use of diabetes products
- To check and assess injection technique of patient
- To be able to check and assess injection sites
Non - Clinical
- Keeping Clinic rooms stocked with necessary equipment and literature.
- Ensure clinic rooms are tidy and clean
- Collate Dafne Data as per national Guidance
- Ordering of supplies for outpatients and inpatients
- Organising Dafne sessions
- To undertake any IT processes specific for the running of the diabetes/endocrine clinics
Education and Training/Self-Development
Contribute to in house staff training
- Identify own training and development needs and undertake appropriate training/education as required.
- Participate in an annual individual performance review process where objectives will be agreed, performance monitored and personal development needs discussed.
- To attend all statutory and mandatory training as and when required to do so.
- To work towards the relevant Trend Competencies Framework, unregistered practitioner level as discussed with manager
- Act responsibly in respect of colleagues health, safety and welfare following safe work practices and complying with the Trusts Health and Safety Policies.
- Adhere to all Trust Policies as applicable.
