Job summary
he Band 6 Clinical Streamer plays a pivotal role in ensuring safe, effective, and timely patient flow within the Urgent Treatment Centre (UTC). Working as an autonomous, highly skilled practitioner, the post holder is responsible for the rapid clinical assessment, risk stratification, and appropriate streaming of patients to the most suitable care pathway. This includes directing patients to GP-led urgent care, minor injury/illness management, emergency care services, community alternatives, or self-care where appropriate.
Acting as the first point of clinical contact, the Clinical Streamer ensures early identification of acutely unwell or deteriorating patients, initiates immediate interventions where required, and supports the delivery of high-quality, patient-centred care. The role involves collaborative working with multidisciplinary teams, including GPs, Emergency Nurse Practitioners/Advanced Nurse Practitioners, UTC nursing teams, Emergency Department staff, ambulance services, and community providers to optimise patient safety and throughput.
The Band 6 Clinical Streamer is expected to demonstrate clinical decisionmaking skills, sound clinical judgement, and the ability to work effectively within a fast-paced environment. The role also contributes to service development, clinical governance, and continuous quality improvement within the UTC setting.

The post holder will be a Registered Nurse with a current NMC Registration responsible for appropriate clinical streaming of Adults & Children patients attending the Urgent Treatment Centre and being professionally and legally accountable for effective streaming of patients into acute and community primary care services.
The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will have a commitment to collaborative working and actively support and liaise with other health and social care professionals and agencies.
The post holder will be able to carry out a physical assessment including all first line diagnostics. They are expected to demonstrate and apply a broad range of nursing knowledge, interventions and skills. Knowledge of the principles of health promotion and preventing illness will be fundamental as will having a comprehensive understanding of the roles of others in health and social care.
The Urgent Treatment Centre provides a 7-day a week, 24-hours a day service for 365 days a year the postholder will be required to work Day & Night shifts patterns , as operationally required with the aim to avoid hospital admission through clinical assessment or streaming to primary care/acute services.

Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
High Cost Area Supplement – Outer London (B&D, Redbridge, Havering and Waltham Forest, CEME) This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).
