Job summary

Band 7 Community Diabetes Nurse An exciting opportunity has arisen for a Band 7 Community Diabetes Nurse to work with our Integrated Diabetes Nursing service in the borough of Barking and Dagenham and the wider BHR teams. We are looking for an enthusiastic and motivated individual, with excellent communication skills, who is able to work autonomously, as part of the team and to be an integral member of the wider Diabetes teams in supporting people managing the complexities of diabetes management. This role requires facilitating patient structured education , having a sound knowledge of the principles of diabetes management and care and a keen interest in professional development & advancing with technology. Therefore experience of teaching and good IT skills is essential. The post holder will be responsible for providing; supporting and developing the delivery of high quality; evidence based Diabetes clinical practice throughout the local community, in health centre clinics and patients homes. The successful applicant will be required to work closely with the lead Diabetes consultant, GPs, nursing colleagues and other members of the multidisciplinary team. The ability to travel effectively across the boroughs is necessary.

Main duties of the job

To be responsible for organising and prioritising own and others workload in the day to day allocation of work. To work collaboratively with colleagues, or autonomously, to provide the highest possible standard of nursing care for people (adults) with diabetes in the community. To attend and contribute to the Integrated Diabetes Service and multi-disciplinary team meetings across the primary and secondary care setting To represent the Integrated Diabetes Service and/or Specialist Services at appropriate meetings locally, regionally and nationally. To contribute to the development of clinical expertise in diabetes in all areas of the Organisation which provide care for people with diabetes. To assess local GP practices and provide specialist support to improve the standards and delivery of diabetes care To take an active role in delivering Type 2 Structured Education and be an accredited educator To deputise when required in the team managers absence and delegate appropriately to health care assistants. To be responsible for providing accurate records of information required by the Trust for audit purposes. To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register.

Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – Outer London (B&D, Redbridge, Havering and Waltham Forest, CEME) This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).
