Job summary

An exciting opportunity has arisen for a Palliative care End of life Facilitator to complement our existing COMPASS team (Community Palliative advise and support service) The post based within Waltham Forest is to support our existing team in raising awareness of local and national EOL initiatives Working closely with Care homes across the borough the successful applicant will develop and maintain links with commissioners and key stakeholders throughout Waltham Forest Working across the borough the successful applicant will support in delivering Education and will act as a point of contact for the Community Nursing Team Applicants are welcome for Registered Practitioners with experience in palliative/End of Life Care

Main duties of the job

To promote and raise awareness of the Local & National End of Life Initiative across Waltham Forest Community Services To develop and maintain links with commissioners and key stakeholders throughout WF To provide facilitation to localities to support implementation and monitoring of end of life initiatives and to promote new ways of working and good practice highlighted by The Gold Standard Framework (GSF) and Advance Care Planning (ACP). To engage and liaise with the Team leader and Nurse Consultant to develop WF wide initiatives on end of life care including the promotion of the end of life care register. To develop a work plan to deliver initiatives and evaluate. To facilitate GPs to identify named co-ordinators within their practice who will set up palliative care disease registers, palliative care team meetings (GSF) and the setting up and maintenance of the electronic palliative care register. To provide on-going support to GP practices, District nurse teams and others implementing end of life initiatives offering practical advice over the phone and individual visits to localities. To provide education, advice, signposting and support for Residential and Nursing homes in the management of their residents approaching end of life. To provide support, education, training and reflective sessions on EOL care for community nursing teams as identified.

Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes, Essex.

Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

 

High Cost Area Supplement – Outer London (B&D, Redbridge, Havering and Waltham Forest) This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,870 to a maximum of £6,137 p.a. pro rata for part time).
