Specialist Palliative Care Practitioner

Job summary

An exciting opportunity has arisen for a palliative care advanced practitioner band 7 to work in the COMPASS (Community Palliative Advice and support service) in Waltham Forest. Applicants will work as an effective and responsible member of the palliative care team, providing a consistently high level of specialist palliative care advice to patients with a life limiting illness. their families and carers throughout the illness journey. You will be required to autonomously managing a caseload of patients, undertaking a comprehensive assessment of individual patient's palliative care needs, working with other teams and communicating effective recommendations to relevant professionals. You will be required to have knowledge of when to re-evaluate, transfer care or discharge from the service.You will work in collaboration with community teams in WF

Applicants will be a Registered practitioner (NMC or HCPC) responsible for patients allocated to them and will assess, manage, plan and deliver care, including being professionally and legally accountable.

The post holder will be expected to work collaboratively with partner organisations, service users, carers, clinicians, and other practitioners within the multi-disciplinary team in delivering services within the community in a widely diverse locality. The Compass team provides a 7-day a week, 8-8 day service 365 days a year.

The post holder will require to work shifts, with the 8-8 period.

Main duties of the job

The post holder will be able to :

  • To act as an expert nurse/Practitioner for patients and families and be a resource for advice and consultancy to empower other professionals to provide high quality palliative/end of life care.
  • To work autonomously to provide holistic assessment and management for patients with complex problems, and their families, enhancing their quality of life and enabling them to make choices about their care. Monitoring, evaluating and reviewing palliative interventions.
  • To act as a patients advocate and support informed discussions around choices concerning treatment options affecting quality of life and preferences around place of care and place of death.
  • To work collaboratively as part of a multi-disciplinary team supporting colleagues and respecting their contribution to meetings.
  • To have organisational knowledge relating to Trust protocols and procedures and adhere to them.
  • To be responsible for providing accurate records of information required by the Trust.
  • To contribute to quality initiatives and service development.
  • To have a sound clinical knowledge on all aspects of palliative care and palliative medicine with awareness of the benefits and reactions to treatments and drugs used for symptomatic and therapeutic benefit, providing expertise in symptom control and offer appropriate advice as required
