We are looking for an experienced Band 6 Physiotherapist to join our well established Integrated Adult Community Rehabilitation Therapy Team.

Barking and Dagenham ICT community service is a forward-thinking and innovative service that works in a fully integrated way, with physiotherapists, occupational therapists, district nurses and community matrons all working side by side to ensure an integrated and holistic approach to the people accessing the community services.
Our community rehab team assesses housebound adults with complex needs within their home environment. By assessing people within their home environment, we are in a good position to significantly improve people's quality of life and promote independence at home by providing therapy ,therefore making this a very rewarding job.

Main duties of the job
- Demonstrate flexibility, good communication skills and ability to work as a team member.
- The ability to travel efficiently across the geographical area covered by NELFT is essential.- To prioritise workload, delegate and to supervise junior staff, support staff and students, to aid provision of high standards of patient care.- To work as an autonomous practitioner in the community, liaising closely with other members of the team, and other services as appropriate.- To carry out specialised Physiotherapy assessments and interventions for patients with complex needs.- To establish, evaluate and modify appropriate treatment plans, recognising own limitations and seeking advice when necessary.- Managing a caseload of patients using evidence based practice, the principles of Occupational Therapy and patient centred practice.- To monitor, evaluate and modify treatment for patients with multiple conditions in order to measure progress and ensure effectiveness of treatment.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process, new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High-Cost Area Supplement - London
This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436).

- To complete comprehensive physiotherapy assessments, through interview, observation of functional tasks and/or use of standardised assessments for patients referred to the service with a range of complex diagnosis and symptoms.
- To be responsible for organising and prioritising own caseload in the day to day allocation of work, whilst being able to work flexibly to meet the needs of their patients, their relatives and carers and the service.
- To work autonomously as a lone worker in the community.
- To deputise when required in your seniors absence and delegate appropriately to more junior members of staff, support workers and rehabilitation assistants with guidance as appropriate.
- To have organisational knowledge relating to Trust protocols and procedures and adhere to them, particularly administration of medicine and moving and handling.
- To be responsible for providing accurate records of information required by the Trust for audit purposes.
- To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register.
- To liaise closely with the multi-disciplinary team, specialist nursing staff, patient, carers and other professionals involved in care provision.
- To respect peoples privacy, dignity and individuality. To initiate and make referrals to GPs, consultants and other service professionals and appropriate organisations in agreement with the patient.
