Band 7 - Care Coordinator - Basildon and Brentwood Integrated Community Team.
This is an expansion of a developed role to support increasingly complex patients in the community that are already on the Integrated Community Team caseload. These would be patients that may have non-healing wounds, are highly vulnerable to self-neglect, safeguarding or capacity issues , complex carer/family and social situations, high users of unplanned services. The role would primarily be a clinical role but not exclusive to a clinical post and we would welcome applications from any sector to apply. We will also require you to assist the Operational Lead and Team Lead in developing new ways of working, improved streamlined processes, improved documentation, risk assessment and care planning. You will also be carrying out an element of management for the team.

The post is fulltime 37.5 hours per week , part time would be considered but a minimum of 22.5 hours per week is essential.
Main duties of the job
We want someone who is able to see our patients as individuals, with multifaceted needs, who has the ability to carry out a high level holistic assessment of all areas of patients lives, including the opinions of their families and carers. You will be responsible for your own workload and allocated patients , working autonomously but with the support of a wider integrated team. Agile working is encouraged to support best practise and safe patient care. You must be able to get yourself and any equipment that you need around our communities , efficiently and independently.
We would need you to have had previous significant experience at Band 6 /7 in a community setting, leadership at team level, be able to work autonomously and proactively, be able to bring ideas to the table and constructively analyse the benefits and negatives of the role and have highly developed communication and negotiating skills.
You will be working with a diverse community enabling you to develop knowledge and skills around differing cultural perspectives. We work in patients own homes and residential homes. Our patients are over the age of 18 with a predominant physical nursing need.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,192 and a maximum payment of £2,011 per annum pro rata).
