Community Learning Disability Nurse (Band 5)
An exciting opportunity has arisen within the Community Learning Disability Team for an enthusiastic Learning Disability Registered Nurse (RNLD) to be a part of the Barking & Dagenham Integrated Care Services.
This position is part of a multi-disciplinary team, working alongside occupational therapists, nurses, psychologists, art therapists, behaviour specialists, psychiatrists and physiotherapists to provide the best care to our service users. We also work closely with our social care colleagues and Primary Care Network.
The post holder will work and contribute to the care of those with learning disabilities, autism and/or additional complex needs. Post holder would develop and promote person centred care approaches and educate and inform clinical and non-clinical staff on best evidence-based care.
The post holder will provide a specialist role within the Community Team and will demonstrate higher levels of standards of care and to ensure that own clinical practice is driven through consistent implementation of National and Trust policies and Clinical Governance requirements.
Main duties of the job
Support and empower clients, their families and carers to influence and participate in decisions concerning their care by providing information on a range of specialist nursing care and service options.
Independently manage a caseload of adults with a diagnosis of Learning Disability, by planning, delivering and reviewing their individualised care and support.
Participate in local and national learning disability mainstreaming and health equality projects as required, which will include the Learning Disability DES, LeDeR, Care Treatment Review Program. Health for all group, and 6 lives.
Actively participate in meetings and strategy meetings to enhance the development and improvement of care and services the team provides. Act as a professional role model providing nursing leadership in the development for junior staff and students. Be accessible and responsive to the needs of service users, staff and carers. Be able to conduct clinical assessments and reviews of service users with learning disabilities, autism, and other physical health conditions. Ensure systems are in place for assessment, care and discharge planning within a multi-disciplinary framework, ensuring the Care Programme Approach or any other statutory method of care delivery works effectively.

Starting with NELFTNELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
