Job summary

Come and be part of one of our newly launched, integrated mental health & wellness teams in Havering. These new teams have merged assessment/brief intervention and recovery teams, removing the need for a two-tier service and no more hand offs of patients from one service to another. We are also working with our teams, carers and service users on how to work with a patient's network. We are only a few months into our transformation so now is a great time to come and influence the change.

We have had new investment in mental health worker posts, peer support workers posts and support time recovery workers to support the integrated team to work as one around the patient, their families and their social networks. Our vision is for this new investment to help start a new era in our mental healthcare provision at NELFT, making us among the most holistic, relational and person-centred services in the country.

Main duties of the job

Our aims include a greater focus on prevention and wellbeing and delivering more integrated and personalised care closer to home. By working with Primary Care Networks (PCNs) and local partners in each borough, NELFT propose to support plans by using a "Neighbourhood Team" model of care to develop multi-agency, integrated and place-based care to support people with serious mental health needs to receive their care closer to home.

The post holder's principal responsibility is to provide a person-centred approach in our service provision, focusing on the needs of the service user and their carers/network.

Responsibility for:

  • Co-ordinating the care of an identified caseload of service users according to the Care Programme Approach (CPA).
  • Acting as Primary Worker to a defined caseload service of users receiving brief interventions.
  • Assessing and planning treatment effectively in partnership with service users, their carers, families and network to identify needs.
  • Undertaking full core assessments, comprehensive risk assessments, developing a well-defined crisis plan, ensuring these are updated on a regular basis.
  • Participate in the Duty & Referrals Management System
  • Work in partnership with the MDT, Home Treatment Team, Adult Social Care, Substance Misuse Services, Psychology Services, Acute Inpatients Services and other services to provide support and treatment to service users in crisis to facilitate the safe management of their needs within the community.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Higher cost area Supplement

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,108 to a maximum of £5,177).
