Job summary

The community treatment team (CTT) works with adults in the community with an acute physical need who could potentially be treated at home, rather than attend the Emergency Department (ED).The aim of the service is to prevent unnecessary hospital admissions for those people who are experiencing an acute, physical health crisis, for example: a suspected infection,falls without injuries, or an exacerbation of along-term condition such as COPD. The service consists of Doctors, nurses, Health care assistants,Occupational therapists, Social workers, Advance Care practitioners, Pharmacists and Physiotherapists who work with primary care, community services and healthcare partners at Queen's Hospitals so that people can receive medical, nursing and therapy interventions in their own homes wherever possible, without need for hospital admission. The post holder should be a highly motivated and enthusiastic nurse leader, who is passionate about delivering high quality, compassionate care and who is equipped with excellent interpersonal and leadership skills. Main duties of the job

Professional Leadership - providing highly specialist expert advice and knowledge in relation to Nursing and AHP delivery of CTT Services across the London Boroughs of Barking, Havering and Redbridge.

Strategic Planning/Policy and Development Education and Training - develop and deliver specialist educational programmes to meet the needs of practitioners and other relevant stakeholders. This shall be achieved by leading in the identification of educational needs; working in partnership with the Education and training directorate in investment planning and developing a framework to meet these educational requirements within the area of responsibility.

Quality improvement- To lead Quality improvement initiatives to support CTT colleagues and other partners to feel confident and competent in their roles to deliver individualised care and outcomes.

Research and Development -be a member of the relevant audit/research steering groups; developing and working with relevant stakeholders to undertake research, proposals, and evaluate. The prime driver shall be the attainment of excellence in clinical practice with the ultimate aim of achieving individual patients & carers wishes and outcomes. Provide expert leadership in implementation of CTT Service Objectives, participating in audits, teaching and developing the workforce by monitoring progress and embedding learning.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process, new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High-Cost Area Supplement - London

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436).
