We are looking for an enthusiastic and energetic Specialist SCPHN - Hard to Reach Community Lead to support vulnerable families in accessing health and community services in B&D. This opportunity will be based in the West Locality.

We are aligned to the with GP Services, work closely with the Family Hubs and other partner agencies.

Main duties of the job

The post holder will be expected to:

  • Support practitioners to develop health care plans in relation to working with complex families within hard to reach group populations
  • Work with the Universal Health team, and other statutory, non-statutory and voluntary agencies, to develop public health activities which are designed to meet the needs of marginalised communities, children and their families in line with key local and national objectives
  • Develop care pathways to meet vulnerabilities
  • Promote targeted intervention with a defined /escalated workload to enable an increased uptake of national screening/preventative services
  • Work with existing services to promote appropriate, ready and equal access to health care e.g. registration with GP practices, Walk-in Centres and Accident and Emergency departments.
  • To establish and develop relationships with other agencies working with this client group e.g. housing, education, asylum seekers and refugee associations, benefits agencies, social care, Safeguarding Children's Team, Family Hubs and other statutory, non-statutory and voluntary agencies.
  • To work with existing services to further develop systems to identify and track vulnerable children within this client group.
  • To work closely with the MASH and Early Intervention Teams, and be the Locality representative on required panels in relation to Early intervention.

About us

Starting with NELFTNELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary PeriodThis post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).High Cost Area Supplement - LondonThis post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum).
