We are looking for an experienced and enthusiastic specialist physiotherapist.
The successful candidate will work with a highly motivated stroke multi-disciplinary team in the South West Essex area
The successful candidate will be supporting the early supported discharge team to enhance the care of community stroke patients.
It is an exciting opportunity for a physiotherapist with a passion and expertise for stroke who excels in strong leadership and ability to work in a multi-disciplinary team.

The post holder will need to demonstrate varied experience of neurological physiotherapy experience, preferably within acute and community stroke settings and to share a passion for progressing stroke rehabilitation services.
Main duties of the job
Stroke Hub service is a forward-thinking and innovative service that works in a fully integrated way, with physiotherapists, occupational therapists, Nurses, Speech and Language Therapist , Dietitian and social services all working side by side to ensure an integrated and holistic approach to the people accessing the community services.
All of our physiotherapy assessments and treatments take place within the persons home environment in the community. This allows us the opportunity to carry out good quality Stroke rehabilitation that truly makes a positive difference to the persons quality of life. Jointly working with social services, OT's and the other members of our integrated team, making this a very rewarding job due the differences we can make in peoples abilities to live as independently as possible at home.

Starting with NELFTNELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,192 and a maximum payment of £2.011 per annum pro rata).

Our stroke Hub Team see newly discharged stroke patients, where rehab is passed directly from the hospital wards to our team to then continue in a goal orientated approach with this rehab that is so essential to ensuring that quality of life and independence is restored within the home environment.
We are a well supported team, we have 4 rehab assistants supporting the team and all the community therapists ensuring your time can be spent with the more complex cases that really need the physiotherapist input. We also have weekly team allocation meetings, which is a supportive meeting to discuss your community caseload with a band 7 therapist as well as with peers and where joint visits can be arranged for support with more complex cases. We ensure that we complete monthly supervisions and yearly appraisals and also support the training needs of all our therapists.

- .To complete comprehensive physiotherapy assessments, through interview, observation of functional tasks and/or use of standardised assessments for patients referred to the service with a range of complex post stroke presentations and symptoms
- To be responsible for organising and prioritising own caseload in the day to day allocation of work, whilst being able to work flexibly to meet the needs of their patients, their relatives and carers and the service.
- To work autonomously as a lone worker in the community.
- To deputise when required in your seniors absence and delegate appropriately to more junior members of staff, support workers and rehabilitation assistants with guidance as appropriate.
- To be responsible for training and supervision of junior staff, students, assistants and carers in the Community.
- To have organisational knowledge relating to Trust protocols and procedures and adhere to them, particularly administration of medicine and moving and handling.
- To be responsible for providing accurate records of information required by the Trust for audit purposes.
- To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register.
- To liaise closely with the multi-disciplinary team, specialist nursing staff, patient, carers and other professionals involved in care provision.
- To respect peoples privacy, dignity and individuality. To initiate and make referrals to GPs, consultants and other service professionals and appropriate organisations in agreement with the patient.
