We are welcoming applicants for bands 4, 5 and 6 - Community Adult Healthcare, South West Essex.
We are seeking people who are kind, caring and compassionate to compliment the South West Essex Community Health teams. Our District Nurses, HCAs and community nurses support the delivery of holistic health care to patients in their own homes.

Main duties of the job
As a member of our community multi-disciplinary team, you will work within a primary care network in a neighbourhood team alongside Team Lead, Community matrons, care coordinators, District Nurses, Specialist Nurses, Therapists and Health Care Assistant's to meet the needs of a complex community delivering patient care supporting early discharge and avoiding admissions to the acute sector.
This includes.
- End of life care,
- Managing the deteriorating patient,
- Complex and simple wound care including VAC therapy and compression therapy,
- Venepuncture,
- Utilising advanced physical assessment skills and many more clinical nursing skills.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement - London
This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,192 and a maximum payment of £2,011 per annum pro rata).

All new starters will join the Trust of the first Monday of each month and will undertake a comprehensive induction of up two weeks which will include mandatory training, systems training and the allocation of equipment. as part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be help at our head office in Rainham, Essex
Community nurses, HCAs and District Nurses, provide high quality care in the community outside of a hospital environment. If youre looking to take up one of our many community nursing roles, youre likely to find yourself as an agile worker based in patients homes and residential care homes.
Being a community nurse or District Nurse, requires good listening skills and the ability to relate to people of all backgrounds.
A passion for care and the patience for teaching are also important qualities within this profession as the work includes educating both patients and family on the basics of healthcare and self-management/shared care.
Professionals in community nurse jobs will work varied shift patterns, of 7.5 hour shifts, to cover the team operating hours from 7am 7pm, visiting patients with a range of physical health conditions.
Our residents will usually include elderly people with health concerns, physically disabled people, including any complex care needs from age 18 upwards including those with terminal illnesses.
The daily workload for a community nurse is wide ranging from assessment, wound and leg ulcer care, diabetes management, many clinical nursing procedures, emotional support to helping patients and families to manage their condition.
