Hepatology Assistant Practitioner - Fixed Term 12 Months

Job summary

An exciting opportunity has arisen to join our Trust as a Band 4 Hepatology Assistant Practitioner on a Fixed Term contract for 12 months. This post would suit an enthusiastic and self-motivated individual who is keen to learn new skills and provide holistic support/management to our local patients with liver disease.

The post holder will drive identification of patients with viral hepatitis, and other aetiologies of liver disease, and support patient pathways through quick access to Fibroscans, liver assessment, and management. This role will be based between our two sites at Hillingdon and Mount Vernon Hospital. There may be a requirement for community working in the future.

The Hepatology Department comprises one Consultant Gastroenterologist and Hepatologist and two Hepatology Clinical Nurse Specialists. The service has developed over the last five years, with a caseload of over 2000 patients with chronic liver disease and over 4000 outpatient contacts per year. The development of the service has contributed to admission avoidance, early supported discharge, and improved patient outcomes.

Main duties of the job

  • Perform FibroScans to support Consultant and Nurse led Clinics, with a possibility of performing FibroScans in alternative care settings in the community .
  • Provide and receive complex and/or sensitive information from patients and their carers and provide information on their test results.
  • Assist with Hepatology investigations, elective admissions and imaging pathway.
  • Organise and perform outreach clinics to deliver care closer to patients such as in the local community alcohol services or clinics/practices.
  • Support the Hepatology team with case finding for hepatitis C and B, identification and engagement of patients into assessment and treatment.
  • Assist the Hepatology team and community partners to engage patients with viral hepatitis treatment, alcohol related liver damage and fatty liver disease.
  • Excellent IT skills, independent data collection and utilise Information Technology, including Excel to high level, and appropriate statistical analysis software to collect and analyse patient data.
  • Provide effective communication with a wide interface of people, both within and outside the Trust, identifying each individual's understanding and comprehension.
