Job summary

An exciting opportunity has arisen to join Rockwood Frailty Unit (On Drayton), Rapid Access Clinic's, as a Band 3 Healthcare Assistant.

You will working independently but along side the clinic Consultant of the day, and other members of the Multidisciplinary team, to deliver high quality care while guiding our patients, and relatives through their outpatient journey.

During each clinic you will be completing patient assessments on Cerner (electronic record keeping), within your remit under the supervision/delegation of the Nurse in Charge/Ward Manager, and carrying out patient care ensuring patient comfort.

You will also maintain privacy, confidentiality, dignity, and ensure the patient is well looked after.

Main duties of the job

  • To independently carry out agreed clinical assessments and activities with the patient including any relevant treatments and dressings within your skill set, obtaining specimens, including blood, or Urine from patients and measuring and monitoring physical characteristics/condition of patients, all under the supervision directions of the Nurse in Charge/ Consultant.
  • To provide support the patients to attend any investigations required during their appointments e.g. CT's, X-Rays, and to facilitate collection of any outpatients medications from pharmacy.
  • To assist the therapists, during their assessments of the patients if required for their appointment.
  • To prepare the patient and assist with clinical procedures, such as wound dressings once assessed by the Nurse in Charge.
  • Serve Snacks/beverages and assist with feeding if required.
  • Weigh, carryout all vital signs including Lying and Standing Blood pressures as per guidelines. Informing the Consultants/Nurse in Charge of any concerns,
  • To ensure all information is clearly documented on Cerner (Electronic Records), to communicate effectively with the care of the elderly secretaries.
  • If clinic is cancelled to work on the Unit to support inpatient team within your shift hours.
