Job summary
The Patient Flow Coordinator is a key part of the Integrated Discharge Team (IDT) working in a patient facing, ward based role. They will work under the direction of the senior members of the IDT and in conjunction with the ward multi-disciplinary teams (MDT).
They will assist with planning and implementing patient discharge; as well as aiding general flow of patientsthrough the hospital
Main duties of the job
SPECIFIC ROLES AND RESPONSIBILITIES:
To be able to work independently as well as with the IT systems interface using initiative and the empowerment of the role to achieve safe and efficient patient flow.
To attend the daily IDT safety huddles and actively participate in the direction and outcomes of those issues identified at the huddle. The PFC must also be part of the ward rounds and board round on their allocated wards.
Promoting criteria led discharges with all members of the MDT
Ensuring that any patient that is medically optimised on their identified wards are placed on the medically optimised list in conjunction with the IDT member allocated to the ward.
Facilitate/assist with checking availability of blood results/ Scan results/referrals to other specialist teams of patients in order to support the patients discharge.
With the guidance from the therapy staff support with any internal and external delays around specific treatment or equipment.
Work with the CSP to inform them of timely discharges, ensuring that theinformation is passed to the relevant Nurse, documented on the Nervecentre and in the patient notes.
In hours the post holder will be responsible for booking transport for non-emergency journeys, in the absence of the ward clerk.
Ensure early escalation of any barriers to discharge to the Matron for the ward area or at the bed meetings.
