Job summary
To provide follow up treatment programs and self-management strategies as prescribed by a qualified member of staff according to patient goals and treatment plans and to raise issues with qualified staff.
To contribute to the delivery of health promotion activities within the service and in accordance with Trust objectives.
To complete onward referrals to appropriate services as advised by qualified staff.
To provide patients with self-management resources as directed by a qualified member of staff.
To gather outcome measures to assist in monitoring the effectiveness of the service.
To undertake administrative, clerical and other duties that support the effective delivery of therapeutic intervention to patients, and running of the service
Access to advice and support from a a qualified staff member is available as required, either directly or by phone. Clinical work is routinely monitored.
Main duties of the job
1.To assist qualified staff in screening new referrals into the service, uploading onto the system and contacting referring services for missing information.
2.To assist the patients to complete outcome measures pre and post course, recording observations and maintaining and updating the database.
3.To maintain stock of all hand outs be responsible for printing additional as required
4.To be able to continue the exercise/respiratory program prescribed by the physiotherapist and provide visual or written information for patients to follow
5.To assist with the delivery of online classes with a qualified member of staff
6. To assist the wider team in delivering patient centered care
7. To assess and where appropriate order equipment to assist the patient to carry out activities of daily living to maintain independence.
8. Be responsible for preparing and running MDT meetings, ensuring that all patients are discussed in detail and in an appropriate timeframe.
9. Ensure that all patients are effectively tracked through their clinical pathway and are seen within the target waiting times and ensuring appropriate records are maintained
